内乳淋巴结放疗研究进展

Review of internal mammary node irradiation in breast cancer

目的对乳腺癌内乳淋巴结放疗的临床研究进展和相关毒副作用进行综述。方法以“乳腺癌、放疗、内乳淋巴结、不良反应”为中文关键词,以“breast cancer、irradiation、internal mammary lymph node、adverse effect”为英文关键词,检索PubMed、CNKI数据库2012-2022年的相关文献。纳入标准:乳腺恶性肿瘤;内乳淋巴结放疗;内乳淋巴结放疗相关毒副作用。排除标准:无关和重复文献。根据纳入和排除标准共筛选出49篇高度相关文献。结果依据当前相关研究,对于腋窝淋巴结转移数≥4枚、1~3枚腋窝淋巴结转移且肿瘤位于内侧象限或中央区的患者强烈建议进行内乳淋巴结照射。结论在现代放疗技术的背景下,内乳淋巴结的放疗可以使患者取得临床获益,但是仍需要谨慎选择合适的患者进行放疗。

Objective To review the clinical research progress of internal mammary lymph node radiotherapy for breast cancer and the side effect.Methods The relevant literatures of PubMed and CNKI databases from 2012 to 2022 were searched with"breast cancer,radiotherapy,internal mammary lymph node,adverse effect"as the Chinese key words and"breast cancer,irradiation,internal mammary lymph node,adverse effect"as the English key words.Inclusive criteria included breast cancer;radiation therapy of internal mammary lymph node;side effects related to radiotherapy of internal lymph nodes.Exclusion criteria contained irrelevant and repeated literature.According to the inclusion and exclusion criteria,49 highly-relevant articles were screened.Results According to the current relevant studies,internal mammary lymph node irradiation is strongly recommended to patients with the number of axillary lymph node metastases≥4,1-3 axillary lymph nodes metastasis and the tumor located in the medial quadrant or central area.Conclusion Under the background of modern radiotherapy technology,radiotherapy of internal mammary lymph node can make patients gain clinical benefits,but it is still necessary to carefully select appropriate patients for radiotherapy.
