探讨欣母沛联合缩宫素对前置胎盘剖宫产产妇产后出血的预防效果及对凝血功能的影响

To explore the preventive effect of hemabate combined with oxytocin on postpartum hemorrhage in women with placenta previa cesarean section and the effect on coagulation function

目的探讨卡前列素氨丁三醇注射液(欣母沛)联合缩宫素对前置胎盘剖宫产产妇产后出血的预防效果及对凝血功能的影响。方法选取74例前置胎盘并行剖宫产产妇,按照随机数字表法分为对照组与研究组,每组37例。对照组分娩后予以缩宫素,研究组分娩后予以欣母沛联合缩宫素。比较两组产后出血发生率、产后出血量、手术前后凝血功能及不良反应发生情况。结果研究组产后出血发生率2.70%低于对照组的21.62%,产后2 h及产后24 h出血量均少于对照组,差异显著(P<0.05)。术前,两组凝血酶原时间(PT)、纤维蛋白原(FIB)及D-二聚体(D-D)水平比较,差异无统计学意义(P>0.05);术后24 h,研究组PT指标短于对照组,FIB及D-D水平均高于对照组,差异显著(P<0.05)。两组患者治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论欣母沛联合缩宫素应用于前置胎盘剖宫产产妇能够有效降低产后出血风险,减少产后出血量,改善凝血功能,且不会增加药物不良反应,值得临床推广与应用。

Objective To explore the preventive effect of carprost amine triol injection(Hemabate)combined with oxytocin on postpartum hemorrhage and the efiect on coagulation function in women with placenta previa cesarean section.Methods 74 women with placenta previa and cesarean section were selected and divided into control group and study group according to random number table method,with 37 cases in each group.The control group was given oxytocin after delivery,and the study group was given hemabate combined with oxytocin after delivery.The incidence of postpartum hemorrhage,postpartum blood loss,coagulation function before and after operation and the occurrence of adverse reactions were compared between the two groups.Results The rate of postpartum hemorrhage in the study group was 2.70%lower than that in the control group 21.62%,and the amount of postpartum hemorrhage at 2 h and 24 h was lower than that in the control group,the differences were significant(P<0.05).Before surgery,there were no significant differences in prothrombin time(PT),fibrinogen(FIB)and D-dimer(D-D)levels between 2 groups(P>0.05).24 h after surgery,the PT index of the study group was shorter than that of the control group,and the FIB and D-D levels were higher than those of the control group,the differences were significant(P<0.05).There was no significant difference in the incidence of adverse reactions between 2 groups during treatment(P>0.05).Conclusion The application of hemabate combined with oxytocin in placenta previa cesarean section can effectively reduce the risk of postpartum bleeding,reduce postpartum blood loss,improve coagulation function,and does not increase adverse drug reactions,worthy of clinical promotion and application.
