中医综合疗法治疗外阴硬化性苔藓脾肾阳虚证30例临床观察

Clinical observation on 30 cases of vulvar lichen sclerosus with spleen and kidney yang deficiency syndrome treated by comprehensive therapy of TCM

目的观察中医综合疗法治疗外阴硬化性苔藓脾肾阳虚证的临床疗效。方法将60例外阴硬化性苔藓脾肾阳虚证患者采用随机数字表法分为治疗组和对照组,各30例。治疗组采用中医综合疗法(包括健脾补肾、益气祛瘀中药内服,中药熏洗,中药药膏外敷,会阴局部阿是穴围刺)治疗,对照组予1%丁酸氢化可的松乳膏与维生素E乳膏混合均匀涂抹于会阴病变处。2组均以10 d为1个疗程,3个疗程后比较2组的临床疗效以及治疗前后的临床症状评分。结果治疗组总有效率为93.3%(28/30),对照组为70.0%(21/30),2组比较差异有统计学意义(P<0.05);治疗后2组临床症状评分明显降低,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组降低更明显,与对照组治疗后比较差异有统计学意义(P<0.05)。结论中医综合疗法治疗外阴硬化性苔藓脾肾阳虚证疗效显著,可明显改善患者外阴瘙痒、会阴疼痛、色素脱失及伤口皲裂等症状,值得临床推广应用。

Objectivee To observe the clinical efficacy of TCM comprehensive therapy in treatment of vulvar lichen sclerosus with spleen and kidney yang deficiency syndrome.Methods60 cases of vulvar lichen sclerosus with spleen and kidney yang deficiency syndrome were divided into treatment group and control group according to random number table method,with 30 cases in each group.The treatment group was treated with comprehensive therapy of TCM(including oral administration of traditional Chinese medicine for invigorating the spleen and kidney,supplementing qi and removing blood stasis,fumigation and washing with Chinese medicine,external application of Chinese medicine ointment,and encircling needling at localized Ashi point of the perineum).The control group was treated with 1%hydrocortisone butyrate cream mixed with vitamin E cream evenly and applied to the lesions of perineum.Both groups took 10 days as a course of treatment.After 3 courses of treatment,the clinical efficacy and clinical symptom scores before and after treatment were compared between the two groups.ResultsThe total effective rate was 93.3%(28/30)in the treatment group and 70.0%(21/30)in the control group.There was a significant difference between the two groups(P<0.05).After treatment,the clinical symptom scores of the two groups decreased significantly,which was statistically significant compared with that of the same group before treatment(P<0.05),and the decrease was more significant in the treatment group,which was statistically significant compared with that of the control group after treatment(P<0.05).Conclusion It has marked clinical efficacy,can significantly improve the symptoms such as vulvar pruritus,perineal pain,depigmentation,chapped wound,etc.to use comprehensive therapy of TCM in the treatment of vulvar lichen sclerosus with spleen and kidney yang deficiency syndrome,which is worthy of clinical promotion and application.
