摘要
目的探讨血清胱抑素C(Cys C)与β2微球蛋白(β2-MG)联合检测在诊断妊娠期高血压疾病(HDP)早期肾损伤中的价值。方法取本院产科从2018年4月-2019年6月收治的68例HDP患者(实验组)和75例正常妊娠孕妇(对照组),分别在妊娠20~24周及30~34周检验二组的血清Cys C、β_(2)-MG、BUN(尿素氮)与Cr(肌酐),并进行组间对比。结果实验组妊娠30~34周的Cys C、β_(2)-MG、BUN与Cr检验值均较妊娠20~24周的大(P<0.05);实验组Cys C、β_(2)-MG在妊娠20~24周和30~34周均大于对照组(P<0.05);在妊娠20~24周时,实验组BUN与Cr与对照组无统计学差异(P>0.05),而在妊娠30~34周时,实验组BUN与Cr均大于对照组,有统计学差异(P<0.05);实验组妊娠30~34周的Cys C、β_(2)-MG、Cys C+β_(2)-MG阳性率均高于孕20-24周,有统计学差异(P<0.05);在妊娠20~24周及30~34周时,实验组Cys C、β_(2)-MG、Cys C+β_(2)-MG阳性率均较对照组大,有统计学差异(P<0.05)。结论血清CysC与β_(2)-MG联合检测对诊断HDP早期肾损伤具有较大的诊断价值,可应用于临床。
Objective To investigate the value of combined detection of serum cystatin C(Cys C)and beta 2 microglobulin(beta 2-MG)in the diagnosis of early renal injury in hypertensive disorders of pregnancy(HDP).Methods 68 HDP patients(experimental group)and 75 normal pregnant women(control group)admitted to obstetrics department of our hospital from April 2018 to June 2019 were examined for serum Cys C,beta 2-MG,BUN(urea nitrogen)and CR(creatinine)at 20~24 and 30-34 weeks of gestation,respectively,and compared between groups.Results The Cys C,beta 2-MG,BUN and Cr test values of the experimental group at 30-34 weeks of gestation were larger than those at 20~24 weeks of gestation(P<0.05);Cys C beta 2-MG of the experimental group in 20~24 weeks of gestation and 30~34 weeks of gestation were greater than the control group(P<0.05);at 20~24 weeks of gestation,BUN and C r in the experimental group were not statistically different from those in the control group(P>0.05),but at 30-34 weeks of gestation,BUN and Cr in the experimental group were greater than those in the control group,with statistical differences(P<0.05);Cys C,beta 2-MG,Cys C+β2-MG in the experimental group at 30~34 weeks of gestation were higher than those of the control group at 20~24 weeks of gestation,with statistical difference(P<0.05);at 20~24 weeks of gestation and 30-34 weeks of gestation,the positive rates of Cys C,beta 2-MG,Cys C+beta 2-MG in the experimental group were greater than those in the control group,with statistical difference(P<0.05).Conclusion The combined detection of serum Cys C and beta 2-MG has great diagnostic value in the diagnosis of early renal injury in HDP and can be applied in clinical practice.
