B超定位下振动排痰对ICU肺部感染患者的应用研究

摘要 目的探讨B超定位下振动排痰在ICU肺部感染患者中的应用价值。方法选取2018年9月-2021年9月深圳市盐田区人民医院ICU收治的60例合并肺部感染患者,随机分为观察组及对照组,观察组30例行B超定位下振动排痰,对照组30例行常规机械振动排痰,对两组干预结果进行比较。结果观察组排痰总有效率为96.67%,较对照组80.00%高(P<0.05)。干预前,两组CPIS、APACHEⅡ评分差异不显著(P>0.05);干预后,观察组各评分较对照组低(P<0.05)。观察组机械通气时间、ICU住院时间较对照组短,ICU住院费用、机械通气人次较对照组少(P<0.05)。观察组病死率为0,明显低于对照组13.33%(P<0.05)。结论B超定位下振动排痰在肺部感染患者中应用价值较高,可减轻患者病情,改善预后,缩短机械通气及住院时间,减轻患者医疗费用,还可降低病死率,值得应用。 Objective To explore the application value of vibration sputum drainage under B-ultrasound in ICU patients with pulmonary infection.Methods 60 patients with pulmonary infection admitted to the ICU of Shenzhen Yantian District People’s Hospital from September 2018 to September 2021 were randomly divided into the observation group and the control group.30 patients in the observation group received vibration sputum removal under the positioning of B-ultrasound and 30 patients in the control group received conventional mechanical vibration sputum removal.The intervention results of the two groups were compared.Results The total effective rate of expectoration in the observation group was 96.67%,higher than 80.00%in the control group(P<0.05);Before the intervention,there was no significant difference in CPIs and ApacheⅡscores between the two groups(P>0.05).After the intervention,the scores of the observation group were lower than those of the control group(P<0.05);The duration of mechanical ventilation and ICU hospitalization in the observation group were shorter than those in the control group,and the ICU hospitalization expenses and mechanical ventilation person times were less than those in the control group(P<0.05);The mortality of the observation group was 0,which was significantly lower than that of the control group(13.33%,P<0.05).Conclusion The application value of vibration sputum drainage under B-ultrasound positioning in patients with pulmonary infection is high.It can reduce the patient’s condition,improve the prognosis,shorten the mechanical ventilation and hospitalization time,reduce the patient’s medical expenses,and reduce the mortality.
