复合舒芬太尼椎管内麻在椎间孔镜手术中应用效果分析

Application Effect Analysis of Compound Sufentanil Intraspinal Anesthesiain Foraminal Endoscopic Surgery

目的:分析复合舒芬太尼椎管内麻在椎间孔镜手术中应用效果,以为提升患者手术质量与手术镇痛效果提供有效参考。方法:选取2019年12月—2021年12月在我院接受椎间孔镜微创手术患者100例,采用随机数字表法分为对照组和研究组,每组各50例。对照组患者在术中给予患者局部麻醉,研究组患者则给予低浓度罗哌卡因与舒芬太尼进行椎管内麻醉。对比并观察两组患者手术切皮时(T0)、手术中(手术进行45min时,T1)、手术缝皮时(T2)的视觉模拟疼痛评分(VAS疼痛评分)的变化情况,同时以改良Bromage评分测定下肢运动阻滞程度,此外,观察两组患者手术后恶心、呕吐、眩晕、心悸等并发症的发生率。结果:研究组患者在接受手术的各时间段内,其疼痛得分显著低于对照组患者(P<0.05),同时研究组患者改良Bromage评分显著低于照组患者(P<0.05),此外,研究组患者的术后并发症总体发生率显著低于对照组患者(P<0.05)。结论:在椎间孔镜手术中实施复合舒芬太尼椎管内麻,可以减轻患者术处疼痛感与下肢运动阻滞程度,同时可以显著降低术后并发症的发生几率,安全性较好,总体效果较为理想,值得推广与实践。

Objective:To analyze the application effect of compound sufentanil intraspinal anesthesia in foraminal endoscopic surgery,so as to provide effective reference for improving the quality of surgery and the effect of surgical analgesia.Methods:A total of 100 patients who underwent foraminal endoscopic minimally invasive surgery in our hospital from December 2019 to December 2021 were selected and divided into control group and study group by random number table method,with 50 cases in each group.Patients in the control group were given local anesthesia during the operation,while patients in the study group were given low concentration of ropivacaine and sufentanil for spinal anesthesia.The visual analogue pain score(VAS)was compared and observed between the two groups at the time of skin incision(T0),during surgery(T1 at 45 minutes after surgery),and at the time of skin suture(T2).The modified Bromage score was used to determine the degree of lower limb motor block.The incidence of postoperative complications such as nausea,vomiting,vertigo and palpitation were observed in the two groups.Conclusions:The pain score of patients in the study group was significantly lower than that in the control group(P<0.05),and the modified Bromage score of patients in the study group was significantly lower than that in the control group(P<0.05).In addition,the overall incidence of postoperative complications in the study group was significantly lower than that in the control group(P<0.05).Conclusion:The application of compound sufentanil in foraminal endoscopic surgery can reduce the pain and lower limb motor block,and significantly reduce the incidence of postoperative complications.The safety is good,and the overall effect is ideal,which is worthy of promotion and practice.
