叙事医学在年轻恒牙牙髓治疗中的应用

Application of Narrative Medicine in Pulp Treatment of Young Permanent Teeth

目的探讨叙事医学在年轻恒牙牙髓坏死治疗中的应用策略,及在患者心理干预方面的效果。方法选取本院2019年6月-2020年6月接收的年轻恒牙牙髓治疗患者120例,并将其随机分为两组,各60例。一组作为对照组,采用常规心理干预模式,一组作为观察组,在常规心理干预的基础上,引入叙事疗法。分析两组患者焦虑、抑郁、疼痛改善情况,健康宣教知晓率,生活质量、患者治疗满意度以及牙髓治疗后并发症发生率。结果两组患者在接受心理干预后,其焦虑、抑郁、疼痛评分均出现下降(P<0.05),且观察组患者三项评分远低于对照组(P<0.05);两组患者接受心理干预的过程中,均涉及健康宣教,观察组患者健康宣教知晓合格率远高于对照组(P<0.05);两组患者接受干预后,观察组生活质量评分明显高于对照组(P<0.05);两组患者接受心理干预后,观察组患者治疗满意率,远高于对照组(P<0.05);牙髓经过治疗后,观察组患者并发症发生率11.67%低于对照组患者并发症发生率28.33%(χ^(2)=8.674,P=0.003)。结论在年轻恒牙患者常规心理干预的基础上,引入叙事疗法,能够有效改善患者的不良感受,提升患者的治疗依从性,改善患者生活质量,提高患者的治疗满意度,且能有效规避牙髓治疗后并发症发生率。

Objective To explore application strategy of narrative medicine in treatment of pulp necrosis of young permanent teeth and effect of psychological intervention on patients.Methods The paper chose 120 young permanent teeth pulp patients in our hospital from June 2019 to June 2020,and divided them into two groups randomly,60 cases in each group.One group served as control group,and the other as observation group.Narrative therapy was introduced on the basis of routine psychological intervention.Improvement of anxiety,depression and pain,awareness rate of health education,life quality,patient satisfaction degree with treatment and incidence of complications after dental pulp treatment was analyzed.Results After psychological intervention,scores of anxiety,depression and pain in two groups decreased(P<0.05).During process of psychological intervention,both groups of patients involved in health education.Qualified rate of health education awareness in observation group was much higher than control group(P<0.05).After intervention,life quality scores in observation group were significantly higher than control group(P<0.05).After psychological intervention,satisfaction degree of observation group was much higher than control group(P<0.05).After dental pulp treatment,complication rate of patients in observation group was 11.67%,lower than control group(28.33%)(χ^(2)=8.674,P=0.003).Conclusion on the basis of routine psychological intervention for young permanent teeth patients,introduction of narrative therapy can improve bad feelings,enhance treatment compliance effectively,improve life quality,treatment satisfaction degree,and avoid incidence of complications after dental pulp treatment effectively.
