二维高频及彩色多普勒超声检查在小儿髋关节一过性滑膜炎中的诊断价值分析

摘要 目的:探讨对小儿髋关节一过性滑膜炎患儿应用二维高频与彩色多普勒超声的诊断价值。方法:选取2020年8月-2022年8月玉溪市中医医院收治的120例髋关节一过性滑膜炎患儿作为研究对象,应用二维高频与彩色多普勒超声进行检查,观察患儿髋关节滑膜内血流和供血情况。比较所有患儿患侧与健侧血流速度的差异。结果:二维高频显示,120例患儿均伴有单侧股骨颈前间隙,包括39例关节囊肿胀,81例合并髋关节积液,所有患儿中均为单侧患病,前隐窝内无回声区,前后直径>5 mm,或者健侧>3 mm,髋关节周围软组织未出现骨质声像图的变化。内回声清晰,可见点状高回声,且关节囊表面光滑。彩色多普勒超声结果显示:(1)患侧:彩色多普勒血流显像分布在0~3级,其中0级占比18.03%,1级占比22.95%,2级占比50.00%,3级占比9.02%。(2)健侧:彩色多普勒血流显像分布在0~3级,其中0级占比44.92%,1级占比29.66%,2级占比25.42%,无3级。患侧髋滑膜内平均血流速度为(13.37±4.16)cm/s,健侧平均速度为(5.28±3.15)cm/s,两组差异有统计学意义(P<0.05)。结论:二维高频、彩色多普勒超声应用在髋关节滑膜炎中的诊断效果确切,不仅能够准确检测滑膜内的血供变化,还能够发现滑膜嵌顿症,对于临床治疗和预后具有较高价值。 Objective:To explore the value of two-dimensional high-frequency and color Doppler ultrasound in the diagnosis of transient hip synovitis in children.Methods:A total of 120 children with transient hip synovitis who were admitted to Yuxi Municipal Hospital of Traditional Chinese Medicine from August 2020 to August 2022 were selected as study subjects.Twodimensional high-frequency and color Doppler ultrasound was used for examination.The blood flow and blood supply in the hip joint synovium of children were observed.The difference in blood flow velocity between the affected side and the healthy side was compared.Results:Two-dimensional high frequency showed that,all 120 children had unilateral anterior femoral tibial space,including 39 cases of joint capsule swelling and 81 cases of joint effusion.All the children had unilateral disease,with no echo area in the anterior recess,the anterior-posterior diameter was>5 mm,or the healthy side was>3 mm,and the soft tissue around the hip joint did not show bone sonographic changes.The internal echo was clear,punctate hyperecho was visible,and the surface of the joint capsule was smooth.Color Doppler ultrasound results showed that:(1)for the affected side:color Doppler flow imaging(CDFI)was distributed in Grades 0~3,among which Grade 0 accounted for 18.03%,Grade 1 accounted for 22.95%,Grade 2accounted for 50.00%,Grade 3 accounted for 9.02%.(2)For the healthy side,CDFI was distributed in Grades 0~3,among which Grade 0 accounted for 44.92%,Grade 1 accounted for 29.66%,and Grade 2 accounted for 25.42%,without Grade 3.The blood flow velocity in the hip joint synovium at the affected side was(13.37±4.16)cm,and that at the healthy side was(5.28±3.15)cm,the difference was statistically significant between two groups(P<0.05).Conclusion:The application of two-dimensional highfrequency and color Doppler ultrasound in the diagnosis of hip synovitis has a definite effect,which can not only accurately detect the blood supply changes in the synovium,but also detect the synovial interposing.It has a high value for clinical treatment and prognosis.
