预住院管理模式的探索与效果分析

Exploration and Effect Analysis of Pre-hospitalization Management Mode

目的:分析天津医学科大学肿瘤医院预住院管理模式的应用背景,包括预住院管理流程改造、医院信息系统改造等,探讨实施预住院模式取得的效果。方法:采用整群随机抽样法抽取外科3个月内入院的全部患者,观察组为通过预住院模式转正式住院,对照组为通过非预住院模式入院,观察两组应用效果。结果:实行预住院模式后,观察组总住院时间、术前住院时间、住院总费用、药费、诊查费、护理费均低于对照组,差异有统计学意义(P<0.05)。结论:预住院患者在正式入院前进行各项常规检查和术前检查,能够缩短患者住院时间,降低医疗费用,提高医院运营效率,有效缓解医疗资源紧张的现状,对解决“看病难、看病贵”的问题有一定指导意义,值得更大范围推广实施。

Objective:To analyze the application background of the pre-hospitalization management mode in Tianjin MedicalUniversity Cancer Hospital,including the pre-hospitalization management process transformation and hospital information systemtransformation,and to explore the effects of implementing the pre-hospitalization mode.Methods:A cluster random samplingmethod was used to select all patients hospitalized in surgery department within 3 months.The observation group was transferredto formal hospitalization through pre-hospitalization mode,while the control group was admitted through non-pre-hospitalizationmode.The application effect in two groups was observed.Results:After the implementation of pre-hospitalization mode,the totallength of hospital stay,preoperative length of hospital stay,total hospitalization expenses,drug expenses,examination expensesand nursing expenses in the observation group were lower than those in the control group,with a statistically significant difference(P<0.05).Conclusion:Various routine examinations and preoperative examinations of pre-hospitalized patients before formaladmission can shorten the length of hospital stay,reduce medical expenses,improve the efficiency of hospital operation,effectivelyalleviate the current situation of medical resource tension,and have a certain guiding significance for solving the problem of"difficulty and high cost of getting medical service",which is worthy of wider promotion and implementation.
