液体敷料治疗卧床患者失禁相关性皮炎的效果观察

Effect observation of Liquid Dressing in Treating Incontinence-Related Dermatitis in Bed-Bound Patients

目的:探讨液体敷料对卧床患者失禁相关性皮炎(IAD)的治疗效果。方法:选取2019年12月-2021年11月南宁市第一人民医院创面修复科收治的128例卧床IAD患者为研究对象,根据治疗方法的不同分为液体敷料治疗组和山茶油治疗组。比较两组IAD治愈情况、治愈所需时间;根据性别、年龄、失禁物、IAD严重程度、渗出量、细菌培养结果、血红蛋白水平及白蛋白水平,观察分析不同疗法对不同情况患者IAD预后的影响。结果:液体敷料治疗组治愈率高于山茶油治疗组,差异有统计学意义(P<0.001)。在男性患者、老年人(>65岁)、失禁物为小便、中等渗出量、IAD严重程度为中度与重度、细菌培养阴性及白蛋白水平低下(≤30 g/L)的患者中,液体敷料治疗组的治愈率高于山茶油治疗组,差异有统计学意义(P<0.05);无论是在血红蛋白水平较高(>90 g/L)或较低(≤90 g/L)的患者中,液体敷料组的IAD治愈率皆明显高于山茶油组,差异有统计学意义(P<0.05)。结论:液体敷料对治疗IAD具有重要价值,与传统的山茶油治疗比较,能够提高治愈率,特别是在男性、老年人、失禁物为小便、IAD较为严重、细菌培养阴性、合并有低蛋白血症的患者中,具有较大的治疗优势。

Objectives:To explore the therapeutic effect of liquid dressing on incontinence-associated dermatitis (IAD) of bedbound patients.Methods:A total of 128 bed-bound patients with IAD who were treated in Wound Repair Department of First People’s Hospital of Nanning from December 2019 to November 2021 were selected as the study subjects.According to different treatment methods,the patients were divided into liquid dressing treatment group and camellia oil treatment group.The IAD cure condition and cure time of the two groups were compared.The patients were divided into subgroups according to gender,age,incontinence,IAD severity,exudation amount,bacterial culture results,hemoglobin level and albumin level,and the effects of different therapies on IAD outcome of patients with different conditions were analyzed within the subgroups.Results:The cure rate in liquid dressing treatment group was higher than that in camellia oil treatment group,and the difference was statistically significant (P<0.001).In male patients,elderly patients (>65 years old),and patients with incontinence of urination,moderate exudation amount,moderate and severe IAD severity,negative bacterial culture and low albumin level (≤30 g/l),the cure rate of IAD in the liquid dressing treatment group was higher than that in the camellia oil treatment group,and the difference was statistically significant (P<0.05).In patients with high (>90 g/l) or low (≤90 g/l) hemoglobin level,the cure rate of IAD in liquid dressing group was significantly higher than that in camellia oil group,and the difference was statistically significant (P<0.05).Conclusion:Liquid dressing has crucial value in the treatment of IAD.Compared with traditional camellia oil treatment,it can increase cure rate,and has great treatment advantage especially in male,elderly,incontinence of urination,severe IAD,negative bacterial culture,and hypoproteinemia patients.
