血府逐瘀汤合升降散加减治疗瘀血阻络型偏头痛的临床效果观察

Clinical Effect Observation of Xuefu Zhuyu Decoction and Modified Shengjiang Powder in Treatment of Migraine with Syndrome of Static Blood Blocking Collaterals

目的:观察血府逐瘀汤合升降散加减治疗瘀血阻络型偏头痛的临床效果。方法:选取2020年3月-2022年3月北京瑞慈医院内科收治的100例瘀血阻络型偏头痛患者作为研究对象,按照随机数字表法分为对照组与治疗组,各50例。对照组采用基础性西药治疗,治疗组在对照组基础上采用血府逐瘀汤合升降散加减治疗。比较两组治疗前后偏头痛症状评分、颅内动脉血流速度及治疗效果。结果:治疗后,治疗组偏头痛疼痛程度评分、疼痛时间评分、疼痛频率评分及总分均低于对照组,差异有统计学意义(P<0.05)。治疗后,治疗组椎动脉、基底动脉、大脑前动脉、大脑中动脉、大脑后动脉血流速度均低于对照组,差异有统计学意义(P=0.000 0)。治疗组治疗总有效率高于对照组,差异有统计学意义(P=0.0144)。结论:血府逐瘀汤合升降散加减对瘀血阻络型偏头痛患者的治疗效果显著,能够减缓颅内动脉血流速度,缩短头痛发作时间,降低头痛发作频率,缓解头痛程度。

Objective:To observe the clinical effect of Xuefu Zhuyu Decoction and modified Shengjiang Powder in the treatment of migraine with syndrome of static blood blocking collaterals.Methods:A total of 100 patients of migraine with syndrome of static blood blocking collaterals admitted to the Department of Internal Medicine of Beijing Ruici Hospital from March 2020 to March2022 were selected as the study subjects,and they were divided into control group and treatment group according to the random number table method,with 50 cases in each group.The control group was treated with basic western drugs,and the treatment group was treated with Xuefu Zhuyu Decoction and modified Shengjiang Powder on the basis of the control group.The migraine symptom score,intracranial arterial blood flow velocity and treatment effect before and after treatment were compared between the two groups.Results:After treatment,the cephalagra pain degree score,pain time score,pain frequency score and total score in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the vertebral artery,basilar artery,anterior cerebral artery,middle cerebral artery and posterior cerebral artery blood flow velocity in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).The overall effective rate of treatment in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The treatment effect of Xuefu Zhuyu Decoction and modified Shengjiang Powder is significant in patients of migraine with syndrome of static blood blocking collaterals,it can slow down intracranial artery blood flow velocity,shorten headache attack time,reduce headache attack frequency,and relieve pain.
