腹部彩色多普勒超声在子宫肌瘤和子宫腺肌病鉴别诊断中的价值

Value of abdominal color Doppler ultrasound in the differential diagnosis of uterine fibroid and adenomyosis

目的探讨腹部彩色多普勒超声在子宫肌瘤和子宫腺肌病鉴别诊断中的价值。方法选取2020年1月至2022年6月邹平市焦桥中心卫生院收治的80例疑似子宫肌瘤、子宫腺肌病患者,所有患者均进行腹部彩色多普勒超声检查。以病理检查结果为金标准,分析腹部彩色多普勒超声的诊断符合率,并比较两种疾病的子宫动脉血流参数。结果病理检查结果显示,80例患者中子宫肌瘤46例、子宫腺肌病34例;腹部彩色多普勒超声检查出子宫肌瘤43例、子宫腺肌病31例,诊断符合率为93.48%、91.18%。两种疾病的诊断符合率比较,差异无统计学意义(P>0.05)。子宫肌瘤患者的子宫动脉阻力指数(0.52±0.10)低于子宫腺肌病患者(0.76±0.14),子宫动脉搏动指数(1.56±0.22)高于子宫腺肌病患者(1.23±0.17)(P<0.05);而子宫动脉收缩期、舒张期血流速度比较,差异无统计学意义(P>0.05)。结论腹部彩色多普勒超声对子宫肌瘤与子宫腺肌病的鉴别诊断价值显著,可对疾病进行有效鉴别,能够为后续治疗提供重要依据,值得推广。

Objective To explore the value of abdominal color Doppler ultrasound in the differential diagnosis of uterine fibroids and adenomyosis.Methods A total of 80 patients with suspected uterine fibroids and adenomyosis admitted to the Jiaoqiao Central Health Center in Zouping City from January 2020 to June 2022 were selected.All patients underwent abdominal color Doppler ultrasound.With pathological examination results as the gold standard,the diagnostic coincidence rate of abdominal color Doppler ultrasound was analyzed,and the blood flow parameters of uterine artery were compared between the two diseases.Results Pathological examination results showed that 46 cases of uterine fibroids and 34 cases of adenomyosis were found among the 80 patients.Abdominal color Doppler ultrasound showed that 43 cases of uterine fibroids and 31 cases of adenomyosis,and diagnostic coincidence rates were 93.48%and 91.18%.There was no statistically significant difference in the diagnostic coincidence rate of the two diseases(P>0.05).The uterine artery resistance index of patients with uterine fibroids(0.52±0.10)was lower than that of patients with adenomyosis(0.76±0.14),while the uterine artery pulse index(1.56±0.22)was higher than that of patients with adenomyosis(1.23±0.17)(P<0.05).However,there was no statistical difference in the systolic and diastolic blood flow velocity of uterine artery(P>0.05).Conclusion Abdominal color Doppler ultrasound has a significant value in the differential diagnosis of uterine fibroids and adenomyosis,which can effectively differentiate the disease and provide an important basis for subsequent treatment.It is worth popularizing.
