摘要 目的:探究染色体区域维护因子1(CRM1)、输出蛋白5(exportin5)在乳腺癌组织中的表达及其临床意义。方法:2016年6月至2017年6月,行手术切除的乳腺癌组织和癌旁正常组织(距离肿瘤组织≥5 cm)87例,采用免疫组织化学染色法检测CRM1、exportin5蛋白在乳腺癌组织中的表达情况。利用GEPIA数据库中的TCGA数据库分析CRM1、exportin5基因在乳腺癌组织中的表达水平。分析CRM1、exportin5表达情况与乳腺癌患者临床病理参数及术后3年内发生复发或转移的关系;采用Cox回归分析影响乳腺癌患者术后复发转移的危险因素。结果:CRM1、exportin5基因在乳腺癌中的表达水平均高于正常乳腺组织,但差异无统计学意义(P>0.05);乳腺癌组织中CRM1、exportin5蛋白的阳性表达率分别为70.11%(61/87)、66.67%(58/87),明显高于正常乳腺组织中的14.94(13/87)、21.84(19/87)(P均<0.05);CRM1、exportin5蛋白表达水平与乳腺癌患者临床分期、分化程度、淋巴结转移有关(P均<0.05);乳腺癌组织中CRM1、exportin5蛋白表达水平之间呈正相关(P<0.05);CRM1蛋白高表达组乳腺癌患者术后复发转移率为70.43%,明显高于低表达组的37.83%(χ^(2)=11.34,P=0.001);exportin5蛋白高表达组乳腺癌患者术后复发转移率为69.82%,明显高于低表达组的41.89%(χ^(2)=10.80,P=0.001);多因素Cox回归分析显示,分化程度低、淋巴结转移、CRM1高表达、exportin5高表达均是影响乳腺癌患者术后复发或转移的危险因素(P均<0.05)。结论:CRM1、exportin5在乳腺癌组织中均呈高表达,二者表达升高是影响乳腺癌患者术后复发或转移的危险因素。 Objective:To investigate the expression and clinical value of chromosome region maintenance 1(CRM 1)and exportin5 in breast cancer.Methods:From June 2016 to June 2017,87pairs of breast cancer tissues and adjacent normal tissues(more than 5 cm aw ay from tumor tissue)resected in thyroid and breast surgery of our hospital w ere collected,the expression of CRM1 and exportin5 in breast cancer tissues w as detected by immunohistochemistry.The expression levels of CRM1 and exportin5 genes in breast cancer tissues w ere analyzed by TCGA database in GEPIA database.The relationship betw een the expression of CRM 1,exportin5 and clinicopathological parameters,recurrence or metastasis w ithin 3 years after operation w as analyzed;Cox regression analysis w as used to analyze the risk factors of recurrence and metastasis in breast cancer patients.Results:The expression levels of CRM1 and exportin5 in breast cancer were higher than those in normal breast tissue,but the difference w as not statistically significant(P>0.05);the positive expression rates of CRM 1 and exportin5 in breast cancer tissues w ere 70.11%(61/87)and 66.67%(58/87)respectively,w hich w ere significantly higher than those in normal breast tissues 14.94(13/87)and 21.84(19/87)(P<0.05);the expression levels of CRM 1 and exportin5 w ere correlated w ith clinical stage,differentiation degree and lymph node metastasis(all P<0.05);there w as a positive correlation betw een the expression levels of CRM 1 and exportin5 in breast cancer(P<0.05);the recurrence and metastasis rate of breast cancer patients in high expression group of CRM 1 protein w as 70.43%,w hich w as significantly higher than that in low expression group(37.83%)(χ^(2)=11.34,P=0.001);the recurrence and metastasis rate of breast cancer patients in high expression group of exportin5 protein w as69.82%,w hich w as significantly higher than 41.89%in low expression group(χ^(2)=10.80,P=0.001);multivariate Cox regression analysis show ed that low differentiation,lymph node metastasis,high expression of CRM 1 and high of exportin5 w ere risk factors for recurrence or metastasis of breast cancer patients(all P<0.05).Conclusion:Both CRM 1 and exportin5 are highly expressed in breast cancer tissues.The increased expression of CRM 1 and exportin5 is a risk factor for recurrence or metastasis of breast cancer patients.
