平均血小板体积在2型糖尿病肾病患者中临床研究

Clinical study of mean platelet volume in patients with type 2 diabetic nephropathy

目的探究平均血小板体积(Mean platelet volume,MPV)在2型糖尿病肾病患者中的变化情况及其临床意义。方法选择2020年1月至2020年12月就诊于青岛市城阳人民医院内分泌科和肾内科的肾功能正常的2型糖尿病肾病患者88例,根据尿微量白蛋白/尿肌酐比值(Urinary albumin-creatinin ration,ACR)进行分组如下:30mg/g≤ACR≤300mg/g组(B组)47例,ACR>300mg/g组(C组)41例。另选择同期就诊的63例2型糖尿病患者(ACR<30mg/g)为对照组(A组),比较此3组MPV的临床变化并分析其相关性因素。结果B组和C组糖尿病病程、ACR、血小板计数、MPV、血肌酐、总胆固醇、甘油三酯、血尿酸、低密度脂蛋白、C-反应蛋白水平均高于A组,而血白蛋白水平低于A组(P<0.05)。相关性分析显示,MPV与糖尿病病程、C-反应蛋白、ACR、血小板计数、总胆固醇、低密度脂蛋白均呈正相关。多元线性逐步回归分析显示,糖尿病病程、C-反应蛋白是MPV的危险因素。结论MPV与2型糖尿病肾病的发病、进展密切相关,是有效且简便的诊疗方法,值得临床重视、推广。

Objective To retrospectively investigate the changes and clinical significance of mean platelet volume in patients with type 2 diabetic nephropathy.Methods A total of 88 patients with type 2 diabetic nephropathy and normal renal function visiting department of endocrinology and nephropathy of Qingdao Chengyang People′s Hospital were recruited.According to the result of urinary albumin-creatinin ration(ACR),88 patients with diabetic kidney disease were divided into group B(47 patients,30mg/g≤ACR≤300mg/g)and group C(41 patients,ACR>300mg/g).63 patients with type 2 diabetes treated at the same time with ACR<30mg/g were selected as the control group(group A).The changes of MPV in the three groups were compared and the correlation factors were analyzed.Results The values of diabetes duration,ACR,blood platelet count,MPV,Scr,TC,TG,UA,LDL and CRP were higher in the group B and group C than those in the group A,and blood albumin level was lower than that in the group A(P<0.05).Correlation study revealed that the MPV was positively correlated with diabetes duration,ACR,blood platelet count,TC,LDL and CRP.Multivariate regression analysis results showed that diabetes duration and CRP were protective factor for MPV.Conclusion MPV was closely related to the occurrence and development of type 2 diabetic nephropathy.It is a simple and effective diagnostic method,which deserves clinical attention.
