Wiltse肌间隙入路伤椎置钉内固定治疗胸腰椎骨折的效果分析

Analysis on the Effect of Screw Internal Fixation of Injured Vertebrae Through Wiltse Intermuscular Approach in the Treatment of Thoracolumbar Fractures

目的探讨Wiltse肌间隙入路伤椎置钉内固定治疗胸腰椎骨折的效果。方法88例胸腰椎骨折患者随机分为两组。两组均行伤椎置钉内固定治疗,对照组经传统后正中入路,研究组经Wiltse肌间隙入路。比较两组的围术期相关指标、椎体前缘高度比、伤椎矢状面Cobb角。结果研究组的术中出血量、术后引流量均少于对照组,手术时间、住院时间均短于对照组(P<0.05)。术后12个月,两组的椎体前缘高度比均高于术前,伤椎矢状面Cobb角均低于术前(P<0.05);研究组的椎体前缘高度比高于对照组,伤椎矢状面Cobb角低于对照组(P<0.05)。结论Wiltse肌间隙入路伤椎置钉内固定治疗胸腰椎骨折具有创伤小、术后恢复快等优势,可有效恢复患者伤椎的生物力学性能。

Objective To explore the effect of screw internal fixation of injured vertebrae through Wiltse intermuscular approach in the treatment of thoracolumbar fractures.Methods 88 patients with thoracolumbar fractures were randomly divided into two groups.Both groups were treated with screw internal fixation of injured vertebrae.The control group was treated with traditional posterior median approach,and the study group was treated with Wiltse intermuscular approach.The perioperative indicators,anterior vertebral height ratio and sagittal Cobb angle of injured vertebrae were compared between the two groups.Results The intraoperative blood loss amount and postoperative drainage volume of the study group were less than those of the control group,and the operation time and hospitalization time were shorter than those of the control group(P<0.05).At 12 months after surgery,the anterior vertebral height ratios of both groups were higher than those before surgery,and the sagittal Cobb angle of injured vertebrae were lower than those before surgery(P<0.05);The anterior vertebral height ratio of the study group was higher than that of the control group,and the sagittal Cobb angle of injured vertebrae was lower than that of the control group(P<0.05).Conclusions Screw internal fixation of injured vertebrae through Wiltse intermuscular approach has the advantages of less trauma and rapid postoperative recovery,and can effectively restore the biomechanical properties of the injured vertebrae of patients.
