摘要
目的探讨血清降钙素原(PCT)联合乳酸(LAC)检测在重症肺炎(SP)诊断中的应用价值。方法选取我院2020年9月至2021年10月收治的62例SP患者作为观察组,按照感染类型的不同分为细菌感染亚组(n=36)与非细菌感染亚组(n=26);另选取同期45例健康者作为健康对照组。比较各组的血清PCT、LAC水平,分析血清PCT、LAC单独及联合检测对SP的诊断效能。结果观察组的血清PCT、LAC水平均显著高于健康对照组(P<0.05)。细菌感染亚组的血清PCT水平显著高于非细菌感染亚组(P<0.05);但两亚组的LAC水平比较,差异无统计学意义(P>0.05)。血清PCT、LAC联合检测诊断SP的灵敏度、特异度、阳性预测值、阴性预测值均显著高于血清PCT、LAC单独检测(P<0.05)。结论血清PCT联合LAC检测在SP的鉴别诊断、病情评估中有较高的临床应用价值。
Objective To explore the application value of combined detection of serum procalcitonin(PCT)and lactic acid(LAC)in the diagnosis of severe pneumonia(SP).Methods 62 patients with SP admitted to our hospital from September 2020 to October 2021 were selected as the observation group and divided into bacterial infection subgroup(n=36)and non-bacterial infection subgroup(n=26)according to the type of infection.Another 45 healthy subjects were selected as healthy control group.The levels of serum PCT and LAC in each group were compared,and the diagnostic efficacies of single detection and combined detection of serum PCT and LAC were analyzed.Results The levels of serum PCT and LAC in the observation group were significantly higher than those in the healthy control group(P<0.05).The serum PCT level in the bacterial infection subgroup was significantly higher than that in the non-bacterial infection subgroup(P<0.05),but no significant difference was found in LAC level between the two subgroups(P>0.05).The sensitivity,specificity,positive predictive value and negative predictive value of combined detection of serum PCT and LAC in the diagnosis of SP were significantly higher than those of single detection of serum PCT and LAC(P<0.05).Conclusions Combined detection of serum PCT and LAC has high clinical application value in the differential diagnosis and disease evaluation of SP.
