中国老年人认知衰弱发生现状的Meta分析

Prevalence of cognitive frailty in Chinese elderly:a meta-analysis

目的:系统评价中国老年人认知衰弱的发生状况及特征,为制定降低老年人认知衰弱发生率的防治策略提供参考依据。方法:计算机检索CNKI、Wanfang data、CBM、VIP、Web of Science、The Cochrane Library、EMBASE、Pubmed数据库中与中国老年人认知衰弱相关的横断面研究,检索时限为建库至2022年3月。2名研究者独立筛选文献、提取数据进行文献质量评价后,采用Stata 15.0软件进行Meta分析。结果:共纳入17篇横断面研究,包括8581例研究对象,Meta分析结果显示,我国老年人认知衰弱发生率为18%[95%CI(15%,22%)]。亚组分析结果显示,不同研究场所老年人认知衰弱发生率不同,医院、社区、养老机构老年人认知衰弱发生率分别为22%[95%CI(17%,26%)]、9%[95%CI(7%,12%)]、24%[95%CI(17%,30%)]。此外,女性、高龄、文化程度低、体育锻炼少、抑郁、营养不良的老年人认知衰弱发生率更高。结论:我国老年人认知衰弱发生率较高,受人群来源、年龄、性别、文化程度、体育锻炼频次、是否抑郁、营养状况的影响。

Objective:To systematically evaluate the prevalence of cognitive frailty in the elderly patients of China.Methods:CNKI,WANFANGDATA,CBM,VIP,Web of Science,The Cochrane Library,EMBASE and Pubmed databases searched to collect cross-sectional studies related to cognitive frailty in the elderly of China,researching time was set from the establishment of the database to March 2022.Related literatures were screened and extracted independently by two reviewers,the risk of bias of included studies were also assessed.Meta-analysis was then performed using Stata 15.0 software.Results:A total of 17 cross-sectional studies involving 8581 elderly individuals were included.The results of meta-analysis showed that the prevalence of cognitive frailty in the elderly of China was18%[95%CI(15%,22%)].The results of subgroup analysis showed that the incidence of cognitive frailty in the elderly of China was different in different research places.The incidences of cognitive frailty of the elderly in hospitals,communities and nursing institution for the aged were 22%[95%CI(17%)26%],9%[95%CI(7%12%)],24%[95%CI(17%)]respectively.In addition,the incidence of cognitive frailty was higher in female,advanced age,people lacking physical exercise,people with low education,people with depression or malnutrition.Conclusions:The incidence of cognitive frailty in the elderly of China is high,which is affected by the source population,age,sex,education level,frequency of physical exercise,depression and nutritional status.
