神经肌肉电刺激疗法联合核心稳定性训练在脑卒中康复治疗中的应用效果及对运动功能、神经功能的影响 被引量:5

Effect of Neuromuscular Electrical Stimulation Combined with Core Stability Training in Rehabilitation of Stroke and Its Effect on Motor Function and Nerve Function

摘要 目的 探讨神经肌肉电刺激疗法联合核心稳定性训练在脑卒中康复治疗中的应用效果及对运动功能、神经功能的影响。方法 选取2020年1—12月湘南学院附属医院收治的96例脑卒中患者作为研究对象,随机分为观察组(神经肌肉电刺激疗法联合核心稳定性训练)和对照组(常规干预),比较两组干预前后对运动功能、神经功能、血小板活化状态。结果 干预前,两组上肢和下肢Fugl--Meyer评测法(FMA)评分比较差异无统计学意义(P>0.05);干预后,两组上肢和下肢FMA评分较干预前明显上升,且观察组明显高于对照组,差异有统计学意义(P<0.05)。两组干预前改良Barthel指数评定量表(MBI)和Berg平衡量表(BBS)评分比较差异无统计学意义(P>0.05);干预后,两组MBI和BBS评分较干预前明显上升,且观察组MBI和BBS评分明显高于对照组,差异有统计学意义(P<0.05)。两组干预前神经功能恶化(ND)和美国国立卫生研究院卒中量表(NIHSS)评分比较差异无统计学意义(P>0.05);干预后,两组ND和NIHSS评分较干预前明显降低,且观察组ND和NIHSS评分明显低于对照组,差异有统计学意义(P<0.05)。干预前,两组P-选择素(CD62P)、血小板凝溶胶蛋白(GSN)、血小板膜糖蛋白纤维蛋白原受体(PAC-1)比较差异无统计学意义(P>0.05);干预后,两组CD62P、PAC-1较干预前明显降低,且观察组明显低于对照组,两组CSN较干预前明显升高,且观察组明显高于对照组,差异有统计学意义(P<0.05)。结论 神经肌肉电刺激疗法联合核心稳定性训练有助于改善脑卒中患者的症状,提高运动功能,改善神经缺损情况,提高神经功能,患者预后良好,价值突出。 Objective To explore the application effect of neuromuscular electrical stimulation therapy combined with core stability training in the rehabilitation treatment of stroke and its influence on motor function and nerve function. Methods 96stroke patients admitted to the Affiliated Hospital of Xiangnan University from January to December 2020 were selected as the research objects, and randomly divided into the observation group(neuromuscular electrical stimulation therapy combined with core stability training) and the control group(routine intervention). The motor function, neural function and platelet activation status before and after the intervention were compared between the two groups. Results before intervention, there was no significant difference in the FMA scores of upper limbs and lower limbs between the two groups(P>0.05). After intervention, the FMA scores of upper limbs and lower limbs in the two groups increased significantly, and the observation group was significantly higher than the control group(P<0.05).There were no significant differences in MBI and BBS between the two groups before intervention(P>0.05). After intervention, MBI and BBS in the two groups were significantly increased compared with before intervention, and MBI and BBS in the observation group were significantly higher than those in the control group(P<0.05).There were no significant differences in ND and NIHSS scores between the two groups before intervention(P>0.05), but after intervention, the ND and NIHSS scores in the observation group were significantly lower than those in the control group(P<0.05). Before intervention, there were no significant differences in CD62P, CSN and PAC-1 platelet activation indices between the two groups(P>0.05). After intervention, CD62P and PAC-1 in the two groups were lower than before intervention, and the observation group was lower than the control group. CSN in the two groups was significantly higher than before intervention, and the observation group was significantly higher than the control group(P<0.05). Conclusion Neuromuscular electrical stimulation combined with core stability training can improve the symptoms of stroke patients, improve the motor function of patients, improve the situation of nerve defects, effectively maintain the nerve function of patients, patients with good recovery, good prognosis, outstanding value.
