左甲状腺素对甲状腺功能减退症患者甲状腺相关激素水平及血管内皮功能的影响 被引量:2

Effect of Levothyroxine on Thyroid Related Hormone Levels and Vascular Endothelial Function in Patients with Hypothyroidism

摘要 目的探讨左甲状腺素对甲状腺功能减退症患者甲状腺相关激素水平及血管内皮功能的影响。方法选取盘锦辽油宝石花医院2018年1月至2020年5月收治的甲状腺功能减退症患者102例作为研究对象,按随机数字表法分为对照组和观察组,各51例。对照组给予甲状腺片治疗,观察组给予左甲状腺素钠片。比较两组临床治疗效果、治疗前后甲状腺相关激素(促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4))、治疗前后血管内皮功能(一氧化氮(NO)及内皮素-1(ET-1))。结果观察组治愈率高达96.08%,明显高于对照组的56.87%(P<0.05);两组治疗前TSH、FT3、FT4水平比较差异无统计学意义(P>0.05);治疗后,观察组TSH水平显著低于对照组,FT3、FT4水平显著高于对照组,差异有统计学意义(P<0.05);两组治疗前NO、ET-1水平比较差异无统计学意义(P>0.05);治疗后,观察组NO水平明显高于对照组,而ET-1水平明显低于对照组,差异有统计学意义(P<0.05)。结论对甲状腺功能减退症患者采用左甲状腺素,可有效提高其治疗效果,改善甲状腺相关激素表达水平,提高血管内皮功能。 Objective To explore the effect of levothyroxine on thyroid related hormone level and vascular endothelial function in patients with hypothyroidism.Methods 102 patients with hypothyroidism admitted to Panjin Liaoyou Baobaohua Hospital from January 2018 to May 2020 were selected as research objects,and were divided into control group and observation group according to random number table,with 51 patients in each group.The control group was treated with thyroid tablets,and the observation group was treated with levothyroxine sodium tablets.The clinical treatment effect,thyroid related hormones (thyroid stimulating hormone (TSH),free triiodothyronine (FT3),free tetraiodothyronine (FT4)),vascular endothelial function (nitric oxide(NO) and endothelin-1 (ET-1)) before and after treatment were compared between the two groups.Results The cure rate in the observation group was 96.08%,which was significantly higher than that in the control group (56.87%,P<0.05);There was no significant difference in the levels of TSH,FT3and FT4between the two groups before treatment (P>0.05);After treatment,the TSH level in the observation group was significantly lower than that in the control group,and the levels of FT3and FT4were significantly higher than those in the control group (P<0.05);There was no significant difference in the levels of NO and ET-1between the two groups before treatment (P>0.05);After treatment,the level of NO in the observation group was significantly higher than that in the control group,while the level of ET-1 was significantly lower than that in the control group (P<0.05).Conclusion The use of levothyroxine in patients with hypothyroidism can effectively improve the therapeutic effect,improve the expression level of thyroid related hormones,and improve vascular endothelial function.
