血清miR-599、sMR联合Hsp90α检测对早期原发性肝癌的诊断价值

Diagnostic Value of Serum mir-599,SMR and Hsp90αin Early Primary Liver Cancer

目的:分析mi R-599、s MR联合Hsp90α检测对早期原发性肝癌的诊断价值。方法:收集2020年6月至2022年6月于我院收治的原发性肝癌患者76例为PHC组,慢性乙型病毒性肝炎患者82例为CHB组,选择同期于我院查体的健康人群45例为HC组。采用荧光定量PCR检测mi R-599表达量,采用ELISA法定量测定可溶性甘露糖受体(s MR)、血热休克蛋白90α(Hsp90α)。比较分析三组研究对象血清mi R-599、s MR及Hsp90α表达水平的差异,分析肝癌患者血清mi R-599、s MR及Hsp90α与肿瘤临床病理相关性,采用受试者工作特征(ROC)曲线评估血清mi R-599、s MR联合Hsp90α对原发性肝癌及早期原发性肝癌的诊断效能。结果:PHC组和CHB组患者mi R-599表达量明显低于对照组,s MR和Hsp90α水平显著高于对照,PHC组和CHB组各项指标差异具有统计学意义(P<0.05)。PHC组患者血清mi R-599和s MR表达量与Child-Pugh分级、门静脉癌栓、淋巴结转移及CNLC分期具有相关性(P<0.05);PHC组患者血Hsp90α表达量与Child-Pugh分级、肿瘤个数、肿瘤直径、门静脉癌栓、淋巴结转移及CNLC分期存在一定关系(P<0.05)。以非肝癌组(HC+CH患者)为对照组,PHC患者为病例组,进行ROC曲线分析,mi R-599、s MR及Hsp90α单独及联合检测诊断PHC的AUC分别为0.728、0.782、0.765和0.867。以非肝癌组(HC+CH患者)为对照组,PHC患者BCLC分期A级为病例组,进行ROC曲线分析,mi R-599、s MR及Hsp90α单独及联合检测诊断早期原发性肝癌的AUC为0.728、0.782、0.706及0.856。结论:血清mi R-599、s MR及Hsp90α在原发性肝癌中差异表达,血清mi R-599、s MR联合Hsp90α检测诊断原发性肝癌甚至早期原发性肝癌具有较高的价值.

Objective:To analyze the diagnostic value of mir-599,SMR combined with Hsp90αin early primary liver cancer.Methods:76 patients with PHC admitted to our hospital from June 2020 to June 2022 were collected as PHC group,82 patients with chronic hepatitis B as CHB group,and 45 healthy people were selected as HC group.The expression of mir-599 was detected by fluorescence quantitative PCR,the level of SMR was detected by ELISA,and the Hsp90αin serum was detected by chemiluminescence.The expression levels of serum mir-599,SMR and Hsp90αin the three groups were compared and analyzed.The correlation between serum mir-599,SMR and Hsp90αin patients with liver cancer and tumor Clinicopathology was analyzed.The diagnostic efficacy of serum mir-599,SMR and Hsp90αcombined with Hsp90αin primary liver cancer and early primary liver cancer was evaluated by ROC curve.Results:The expression of mir-599 in PHC group and CHB group was significantly lower than that in the control group,and the levels of SMR and Hsp90αwere significantly higher than that in the control group.There was significant difference in various indicators between PHC group and CHB group(P<0.05).The expression of mir-599 and SMR in PHC group was correlated with child Pugh grade,portal vein tumor thrombus,lymph node metastasis and CNLC stage(P<0.05);In PHC group,serum Hsp90αexpression was correlated with child Pugh grade,tumor number,tumor diameter,portal vein tumor thrombus,lymph node metastasis and CNLC stage(P<0.05).Taking non liver cancer group(hc+ch patients)as the control group and PHC patients as the case group,ROC curve analysis was carried out.The AUC of mir-599,SMR and Hsp90αwere 0.728,0.782,0.765 and 0.867 respectively.Taking non liver cancer group(hc+ch patients)as the control group and PHC patients with BCLC stage a as the case group,ROC curve analysis was carried out.The AUC of mir-599,SMR and Hsp90αalone and in combination for the diagnosis of early primary liver cancer was 0.728,0.782,0.706 and 0.856.Conclusion:Serum mir-599,SMR and Hsp90αare differentially expressed in primary liver cancer.The detection of serum mir-599,SMR combined with Hsp90αis of high value in the diagnosis of primary liver cancer and even early primary liver cancer.
