血浆环状RNA hsa_circ_0034398在系统性红斑狼疮中的表达和意义

Expression and significance of plasma circular RNA hsa_circ_0034398 from patients with systemic lupus erythematosus

目的:检测系统性红斑狼疮(SLE)患者血浆环状RNA hsa_circ_0034398的表达水平,探讨其在SLE诊断中的应用价值。方法:应用实时荧光定量聚合酶链反应(RT-qPCR)法检测42例SLE患者、36例正常对照者(HC)和42例类风湿关节炎(RA)患者血浆环状RNA hsa_circ_0034398的表达水平。分析SLE患者血浆环状RNA hsa_circ_0034398的表达水平与实验室指标、治疗的关系。应用受试者工作曲线(ROC)分析血浆环状RNA hsa_circ_0034398,以及联合血浆环状RNA hsa_circ_0034398与常规指标对SLE诊断及鉴别诊断的敏感度及特异度。结果:(1)血浆环状RNA hsa_circ_0034398在SLE疾病组的表达水平明显高于HC和RA疾病对照组,差异具有统计学意义(U=476.5,P=0.0052;U=591.0,P=0.0094);(2)SLE疾病组血浆环状RNA hsa_circ_0034398表达水平与N%(rs=0.3098,P=0.0459)呈正相关,与M%(rs=-0.3068,P=0.0481)呈负相关;(3)SLE疾病组血浆环状RNA hsa_circ_0034398表达水平与治疗相关(P=0.0391);(4)ROC曲线分析显示,SLE疾病组血浆环状RNA hsa_circ_0034398与HC、RA疾病对照组、HC+RA对照组的曲线下面积(AUC)分别为0.685、0.665、0.674;敏感度分别为57.14%、47.62%、57.14%;特异度分别为77.78%、95.24%、82.05%;(5)联合血浆环状RNA hsa_circ_0034398+抗双链DNA抗体(Anti-dsDNA)、联合血浆环状RNA hsa_circ_0034398+抗Sm抗体(Anti-Sm)、联合血浆环状RNA hsa_circ_0034398+Anti-dsD‐NA+Anti-Sm在SLE疾病组与HC组的敏感度分别为80.95%、69.05%、83.33%,均高于血浆环状RNA hsa_circ_0034398(57.14%)、Anti-dsDNA(57.14%)、Anti-Sm(26.19%),特异度均为100%。SLE疾病组联合血浆环状RNA hsa_circ_0034398+白细胞计数+红细胞计数+血小板计数HC、RA疾病对照组、HC+RA对照组的AUC分别为0.864、0.906、0.878,敏感度分别为71.43%、88.10%、73.81%,特异度分别为91.67%、88.10%、92.31%。结论:SLE疾病组血浆环状RNA hsa_circ_0034398明显升高,且与疾病炎症程度以及治疗相关,可能作为SLE的潜在诊断及鉴别诊断标志物,尤其是血浆环状RNA hsa_circ_0034398联合SLE常规指标的诊断效能明显升高。

Objective:To investigate the expression of plasma circular RNA hsa_circ_0034398 from systemic lupus erythema‐tosus(SLE)patients and its role in the diagnosis of SLE.Methods:Expression of plasma circular RNA hsa_circ_0034398 in 42 SLE patients,36 healthy controls(HC)and 42 rheumatoid arthritis(RA)patients controls were detected by real-time quantitative poly‐merase chain reaction(RT-qPCR).Receiver operating curve(ROC)was used to analyze the sensitivity and specificity of plasma circu‐lar RNA hsa_circ_0034398,combined with plasma circular RNA hsa_circ_0034398 and conventional indicators for the diagnosis and differential diagnosis of SLE.Results:(1)The expression of plasma circular RNA hsa_circ_0034398 in SLE patients was significantly increased than that in HC,RA group(U=476.5,P=0.0052;U=591.0,P=0.0094);(2)The expression of plasma circular RNA hsa_circ_0034398 in SLE patients group was postively correlated with neutrophil percentage(rs=0.3098,P=0.0459)and the expres‐sion of plasma circular RNA hsa_circ_0034398 in SLE patients group was negatively correlated with momocyte percentage(rs=-0.3068,P=0.0481);(3)The expression of plasma circular RNA hsa_circ_0034398 in SLE patients group was correlated with treatment(P=0.0391);(4)The area under ROC curve(AUC)of plasma circular RNA hsa_circ_0034398 between SLE patients group and HC,RA patients group,HC+RA group were 0.685,0.665,0.674.The sensitivity of plasma circular RNA hsa_circ_0034398 between SLE pa‐tients and HC group,RA patients group,HC+RA group were 57.14%,47.62%,57.14%.The specificity of plasma circular RNA hsa_circ_0034398 between SLE patients group and HC,RA patients group,HC+RA group were 77.78%,95.24%,82.05%;(5)The sensitivity of combined plasma circular RNA hsa_circ_0034398+anti-double-stranded DNA antibody(anti-dsDNA),combined plas‐ma circular RNA hsa_circ_0034398+anti-Smith antibody(anti-Sm),combined plasma circular RNA hsa_circ_0034398+anti-dsDNA+anti-Smith between SLE patients and HC were 80.95%,69.05%,83.33%,which was superior to plasma circular RNA hsa_circ_0034398(57.14%),Anti-dsDNA(57.14%),Anti-Sm(26.19%).The specificity of combined plasma circular RNA hsa_circ_0034398+autoantibodies was 100%.The AUC of combined plasma circular RNA hsa_circ_0034398+WBC+RBC+PLT between SLE patients group and HC,RA patients group,HC+RA group were 0.864,0.906,0.878.The sensitivity of combined plasma circular RNA hsa_circ_0034398+WBC+RBC+PLT between SLE patients group and HC,RA patients group,HC+RA group were 71.43%,88.10%,73.81%.The specificity of combined plasma circular RNA hsa_circ_0034398+WBC+RBC+PLT between SLE patients group and HC,RA patients group,HC+RA group were 91.67%,88.10%,92.31%.Conclusion:The expression of plasma circular RNA hsa_circ_0034398 was significantly increased in SLE patients,and correlated with the inflammation of SLE and treatment.This study suggests that the plasma circular RNA hsa_circ_0034398 may be a potential diagnosis and differential diagnosis biomarker of SLE pa‐tients,especially combined plasma circular RNA hsa_circ_0034398+common indexs.
