碘化油联合PVA肝动脉化疗栓塞治疗晚期肝癌合并上消化道出血的临床效果

Clinical effect of lipiodol combined with PVA transcatheter arterial chemoembo-lization in the treatment of advanced liver cancer complicated with upper gastrointestinal hemorrhage

目的探究碘化油联合聚乙烯醇(PVA)肝动脉化疗栓塞(TACE)治疗晚期肝癌合并上消化道出血的临床效果。方法将100例晚期肝癌合并上消化道出血患者中采用碘化油TACE治疗的设为对照组(50例),碘化油联合PVA TACE治疗的设为观察组(50例)。比较两组的临床疗效。结果观察组的止血成功率、近期治疗总有效率及18、24个月的生存率高于对照组(P<0.05)。治疗后,观察组的天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)及甲胎蛋白(AFP)水平低于对照组(P<0.05)。观察组的恶心、呕吐发生率低于对照组(P<0.05)。结论碘化油联合PVA TACE治疗晚期肝癌合并上消化道出血患者,能够提高止血成功率及化疗效果,保护患者肝功能,提高远期生存率。

Objective To explore the clinical effect of lipiodol combined with polyvinyl alcohol(PVA)transcatheter arterial chemoembolization(TACE)in the treatment of advanced liver cancer complicated with upper gastrointestinal hemorrhage.Methods Among 100 patients with advanced liver cancer complicated with upper gastrointestinal hemorrhage,those who were treated with lipiodol TACE were set as control group(50 cases),and those treated with lipiodol combined with PVA TACE were set as observation group(50 cases).The clinical effects of the two groups were compared.Results The success rate of hemostasis,the total effective rate of recent treatment and the survival rate of 18 and 24 months in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of aspartate aminotransferase(AST),alanine aminotransferase(ALT),total bilirubin(TBIL)and alpha-fetoprotein(AFP)in the observation group were lower than those in the control group(P<0.05).The incidences of nausea,vomiting in the observation group were lower than those in the control group(P<0.05).Conclusion Lipiodol combined with PVA TACE in the treatment of patients with advanced liver cancer complicated with upper gastrointestinal hemorrhage can improve the success rate of hemostasis and the effect of chemotherapy,protect the liver function of patients,improve the long-term survival rate.
