内镜下电凝与钛夹治疗消化性溃疡出血患者的效果比较

Comparison of the effect of endoscopic electric coagulation and titanium clip in the treatment of patients with peptic ulcer hemorrhage

目的比较内镜下电凝与钛夹治疗消化性溃疡出血患者的效果。方法选取2018年1月至2020年12月我院收治的78例消化性溃疡出血患者作为研究对象,根据治疗方案不同将其分为电凝组(39例,内镜下电凝治疗)和钛夹组(39例,内镜下钛夹治疗)。比较两组的治疗效果。结果两组的治疗总有效率比较,差异无统计学意义(P>0.05)。钛夹组的手术中转率、再出血率低于电凝组(P<0.05);两组的输血量比较,差异无统计学意义(P>0.05)。术后3 d,两组的应激反应指标及炎症因子水平均升高,但钛夹组低于电凝组(P<0.05)。结论内镜下电凝与钛夹治疗消化性溃疡出血均具有较好的效果,但内镜下电凝更易出现再出血和手术中转,且应激反应和炎症反应更重。

Objective To compare the effect of endoscopic electric coagulation and titanium clip in the treatment of patients with peptic ulcer hemorrhage.Methods A total of 78 patients with peptic ulcer hemorrhage admitted in our hospital from January 2018 to December 2020 were selected as the research objects.According to different treatment schemes,the patients were divided into electric coagulation group(39 cases,endoscopic electric coagulation treatment)and titanium clip group(39 cases,endoscopic titanium clip treatment).The therapeutic effect of the two groups were compared.Results There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).The operation conversion rate and rebleeding rate in the titanium clip group were lower than those in the electric coagulation group(P<0.05);there was no significant difference in blood transfusion between the two groups(P>0.05).At 3 d after operation,the levels of stress response indexes and inflammatory factors in the two groups increased,but those in the titanium clip group were lower than the electric coagulation group(P<0.05).Conclusion Endoscopic electric coagulation and titanium clip have good effects in the treatment of peptic ulcer hemorrhage,but endoscopic electric coagulation is more prone to rebleeding and operation conversion,and has more severe stress response and inflammatory response.
