LDH、CK-MB对新生儿呼吸窘迫综合征严重程度的预测价值

摘要 目的探讨血清乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)对新生儿呼吸窘迫综合征(NRDS)严重程度的预测价值。方法按照胸片表现将113例NRDS分为轻度组(n=40)、中度组(n=41)、重度组(n=32)。比较三组的一般资料、实验室指标水平,并分析血清LDH、CK-MB及二者联合对重度NRDS的预测效能。结果三组的LDH水平比较,差异具有统计学意义(P<0.05),且LDH水平随病情加重呈逐渐升高趋势。重度组的CK-MB水平高于中度组及轻度组(P<0.05)。LDH联合CK-MB预测重度NRDS的曲线下面积(AUC)最大,为0.862。结论血清LDH、CK-MB水平随NRDS患儿病情加重而呈逐渐升高趋势,可作为评估NRDS严重程度的生物学指标,两者联合对重度NRDS的预测价值更高。 Objective To investigate the predictive value of lactate dehydrogenase(LDH)and MB isoenzyme of creatinine kinase(CK-MB)for the severity of neonatal respiratory distress syndrome(NRDS).Methods According to the chest X-ray findings,113 cases of NRDS were divided into mild group(n=40),moderate group(n=41)and severe group(n=32).The general data and laboratory indexes levels of the three groups were compared,and the predictive efficacy of serum LDH,CK-MB and their combination on severe NRDS were analyzed.Results The LDH level of the three groups was statistically significant(P<0.05),and the LDH level gradually increased with the aggravation of the disease.The level of CK-MB in the severe group was higher than that in the moderate group and the mild group(P<0.05).The area under curve(AUC)of LDH combined with CK-MB in predicting severe NRDS was the largest,which was 0.862.Conclusion The levels of serum LDH and CK-MB increase gradually with the aggravation of NRDS and they can be used as biological indicators to evaluate the severity of NRDS.The combination of the two had higher predictive value for severe NRDS.
