基于行为转变理论的健康指导结合“三三法则”护理模式对输尿管结石术后患者结石排出及疾病进展恐惧感的影响

Effects of health guidance based on behavior change theory combined with"three-three rule"nursing model on stone discharge and fear of disease progression in patients after ureteral calculi surgery

目的探讨基于行为转变理论的健康指导结合“三三法则”护理模式在输尿管结石术后的应用价值。方法选取2018年3月至2021年3月我院收治的160例输尿管结石手术患者,以随机数字表法将其分为对照组和观察组,各80例。对照组采取常规护理,观察组在常规护理基础上采取基于行为转变理论的健康指导结合“三三法则”护理模式。比较两组的干预效果。结果观察组的结石排出率、结石排净率高于对照组,血尿、膀胱刺激症状发生率低于对照组(P<0.05)。干预后,观察组的疾病进展恐惧简化量表(FoP-Q-SF)评分低于对照组,心理弹性量表(CD-RISC)及健康行为能力自评量表(SRAHP)评分均高于对照组(P<0.05)。结论基于行为转变理论的健康指导结合“三三法则”护理模式可促进输尿管结石术后结石排出,减少血尿、膀胱刺激症状的发生,改善患者的疾病进展恐惧感,提高其心理弹性,促进健康行为改善。

Objective To explore the application value of health guidance based on behavior change theory combined with"three-three rule"nursing model after ureteral calculi surgery.Methods From March 2018 to March 2021,160 patients undergoing ureteral calculi surgery in our hospital were selected and divided into control group and observation group by random number table method,with 80 cases in each group.The control group adopted routine nursing,while the observation group adopted health guidance based on behavior change theory combined with"three-three rule"nursing model on the basis of routine nursing.The intervention effects of the two groups were compared.Results The stone excretion rate and stone clearance rate in the observation group were higher than those in the control group,and the incidences of hematuria and bladder irritation symptoms were lower than those in the control group(P<0.05).After intervention,the score of the Fear of Disease Progression Questionnaire-Short Form(FoP-Q-SF)in the observation group was lower than that in the control group,and the scores of Connor-Davidson Resilience Scale(CD-RISC)and Self-Rated Abilities for Health Practices Scale(SRAHP)were higher than those in the control group(P<0.05).Conclusion The health guidance based on behavior change theory combined with"three-three rule"nursing model can promote the stone discharge after ureteral calculi surgery,reduce the occurrence of hematuria and bladder irritation symptoms,ameliorate patients'fear of disease progression,improve their psychological resilience,and promote the improvement of health behavior.
