院外连续动态监测干预对2型糖尿病患者夜间低血糖发生情况的影响

Effect of out-hosital continuous dynamic monitoring intervention on the occurrence of nocturnal hypoglycemia in patients with type 2 diabetes mellitus

目的探究院外连续动态监测干预对2型糖尿病患者夜间低血糖发生情况的影响。方法选取我院收治的100例2型糖尿病患者,随机将其分为研究组(n=49,院外连续动态监测干预)和对照组(n=51,常规院外护理)。比较两组的空腹血糖(FPG)、餐后2 h血糖(2 h PG)水平、糖化血红蛋白(HbA1C)、夜间低血糖发生率,糖尿病自我管理行为量表(SDSCA)、2型糖尿病患者生活质量评定量表(DMQLS)评分。结果干预后,两组的FPG、2 h PG水平及HbA1C均降低,且研究组低于对照组(P<0.05);干预后,研究组的夜间低血糖发生率低于对照组(P<0.05)。干预后,两组的SDSCA各维度评分及总分均升高,且研究组高于对照组(P<0.05)。干预后,两组的DMQLS各维度评分及总分均升高,且研究组高于对照组(P<0.05)。结论院外连续动态监测干预可有效控制2型糖尿病患者血糖水平,降低夜间低血糖发生率,提高自我管理能力及生活质量,值得临床推广及应用。

Objective To explore the effect of out-hospital continuous dynamic monitoring intervention on the occurrence of nocturnal hypoglycemia in patients with type 2 diabetes mellitus.Methods A total of 100 patients with type 2 diabetes mellitus admitted in our hospital were selected and randomly divided into study group(n=49,out-hospital continuous dynamic monitoring intervention)and control group(n=51,routine out-hospital nursing).The levels of fasting plasma glucose(FPG),2 h postprandial plasma glucose(2 h PG),glycosylated hemoglobin(HbA1C),incidence of nocturnal hypoglycemia,Summary of Diabetes Self Care Activities(SDSCA)and Quality of Life Scale for Patients with Type 2 Diabetes Mellitus(DMQLS)scores were compared between the two groups.Results After intervention,the levels of FPG,2 h PG and HbA1C in the two groups decreased,and those in the study group were lower than the control group(P<0.05);after intervention,the incidence of nocturnal hypoglycemia in the study group was lower than that in the control group(P<0.05).After intervention,the scores of each dimension and total score of SDSCA in the two groups increased,and those in the study group were higher than the control group(P<0.05).After intervention,the scores of each dimension and total score of DMQLS in the two groups increased,and those in the study group were higher than the control group(P<0.05).Conclusion The out-hospital continuous dynamic monitoring intervention can effectively control the blood glucose level of patients with type 2 diabetes,reduce the incidence of nocturnal hypoglycemia,and improve self-management ability and quality of life.It is worthy of clinical promotion and application.
