精准运动处方对抗精神病药物引起的药源性肥胖的干预作用

摘要 目的精神分裂症患者需进行抗精神病药物治疗,部分患者由此引起药源性肥胖(drug-induced obesity,DIO)。本文观察精准运动处方干预对精神分裂症患者药源性肥胖的作用,分析其机制。方法选取2020年6月—2021年12月泉州市第三医院收治的100例首发精神分裂症患者为研究对象,经抽签法将其分为试验组和对照组,每组50例。对照组予以常规治疗,试验组由运动处方师和精神科专业人员进行临床生化和体成分等相关体适能指标评测,根据测评结果为每位患者制订个体化运动处方。通过每周集中锻炼和患者自主锻炼相结合的方式监督处方执行情况。在干预前、干预6个月后、出院1个月后分别收集患者相关生理、生化指标数据进行对比分析。结果在不同干预方式下,两组干预6个月后、出院1个月后的体征指标均有改善,且试验组干预6个月后、出院1个月后的体质量、身体质量指数(body mass index,BMI)、腰臀比、内脏脂肪等级均高于对照组,差异有统计学意义(P<0.05)。结论精准运动处方能部分缓解抗精神病药物引起的DIO,本研究所采取干预措施也可为预防或减缓精神分裂症患者的药源性肥胖和并发症提供方案参考。 Objective Patients with schizophrenia need to be treated with psychiatric drugs,and some patients may cause drug-induced obesity(DIO).This paper observes the effect of precision exercise prescription intervention on drug-induced obesity in patients with schizophrenia,and analyzes its mechanism.Methods A total of 100 patients with manual schizophrenia treated in Quanzhou Third Hospital from June 2020 to December 2021 were selected as the research objects.They were divided into experimental group and control group by lot drawing,with 50 cases in each group.The control group was given routine treatment,and the experimental group was evaluated by clinical biochemistry,body composition and other related physical fitness indicators by exercise formulators and psychiatric professionals.According to the evaluation results,individual exercise prescriptions were formulated for each patient.Supervise the implementation of the prescription through the combination of weekly centralized exercise and patients’autonomous exercise.Relevant physiological and biochemical index data of patients were collected before intervention,6 months after intervention and one month after discharge for comparative analysis.Results Under different intervention methods,the physical signs of the two groups were improved after 6 months of intervention and 1 month after discharge,and the weight,body mass index(BMI),waist hip ratio and visceral fat grade of the test group were higher than those of the control group after 6 months of intervention and 1 month after discharge,with statistical significance(P<0.05).Conclusion Precise exercise prescription can partially alleviate DIO caused by psychiatric drugs;the intervention measures in this study can also provide a reference for the prevention or mitigation of drug-induced obesity and complications in patients with schizophrenia.
