DRG精益管理在病案质量管理中存在的问题及改进措施

Problems and Improvement Measures of DRG Lean Management in Medical Record Quality Management

目的讨论分析开展关于疾病诊断的相应分组(diagnosisrelatedgroups,DRG)精益管理,住院首页的填写质量和对DRG亚组的影响。方法将于右江民族医学院附属医院接受诊治处理的2019年1月—2020年12月筛选出患者200份病案作为研究样本,开展病案样本的随机分组,关于病案达标率、病案评分、病案缺陷指数以及病案缺陷整改率进行观察和对比。并对干预前和干预后的DRG分组情况针对住院病案首页的进行对比。结果研究组的病案达标率明显提高18.00%,研究组的病案评分、病案缺陷整改率均明显增高,差异有统计学意义(P<0.05),研究组的病案缺陷指数明显下降,差异有统计学意义(P<0.05)。结论有效的DRG管理和加强病例填报质量控制,对提高住院病例首页数据质量、确保其在DRG平台上的准确分组、确保医疗安全以及提供客观、精准、顶尖质量的数据至关重要。

Objective To discuss and analyze the impact of diagnosis related grouping(DRG)lean management on the quality of the first page of inpatient medical records and DRG grouping.Methods A total of 200 medical records of patients who were treated in Affiliated Hospital of Youjiang Medical University for Nationalities from January 2019 to December 2020 were selected as research samples,and the medical record samples were randomly divided into two groups.The rate of reaching the standard of medical records,the score of medical records,the defect index of medical records and the correction rate of defects in medical records were observed and compared between the two groups.The DRG grouping on the first page of inpatient medical records before and after the intervention was compared.Results The standard rate of medical records in the study group was significantly increased by 18.00%,the score of medical records and the rectification rate of medical records defects in the study group were significantly increased,and the difference was statistically significant(P<0.05).The defect index of medical records in the study group was significantly decreased,and the difference was statistically significant(P<0.05).Conclusion Effective DRG management and strengthening the quality control of case reporting are essential to improve the quality of inpatient case homepage data,ensure their accurate grouping on the DRG platform,ensure medical safety,and provide objective,accurate and top-quality data.
