以“病理学”为导向的闭环式教学模式在妇科肿瘤超声诊断教学中的应用研究

摘要 目的探讨以“病理学”为导向的以案例为基础的学习(CBL)、以问题为基础的学习(PBL)联合多学科协作组(MDT)的闭环式教学模式在住院医师妇科肿瘤超声诊断教学中的应用价值。方法选取2019年5月至2022年6月该院参与超声医学住院医师规范化培训的住院医师60名,随机分为研究组和对照组,每组30名。研究组采取以“病理学”为导向的CBL、PBL联合MDT的闭环式教学模式,对照组采取基于课堂的学习联合MDT的教学模式。比较两组学员理论知识评分及教学评价。结果研究组学员理论知识考试中妇科恶性肿瘤诊断评分、案例分析评分、总分均明显优于对照组,教学评价中教学认可度、提升妇科肿瘤诊断能力、自学能力培养、随访习惯培养、临床思维培养满意度均明显高于对照组,差异均有统计学意义(P<0.05)。结论以“病理学”为导向的CBL、PBL联合MDT的闭环式教学模式更有利于提升住院医师妇科肿瘤超声诊断的能力,培养学员的临床思维和自学能力,值得推广应用。 Objective To explore the value of the closed-loop teaching mode of CBL+PBL combined with MDT oriented to“pathology”in teaching gynecologic tumor ultrasound diagnosis for residents.Methods Sixty residents who participated in the standardized training of ultrasound medicine residents from May 2019 to June 2022 in Zhongnan Hospital of Wuhan University were selected,and the experimental group(n=30)adopted the closed-loop teaching mode of“pathology”-oriented CBL+PBL combined with MDT,while the control group(n=30)adopted the teaching mode of lecture based learning combined with MDT.The participants'theoretical knowledge scores and teaching evaluations were compared between the two groups.Results In the theoretical knowledge examination,the participants in the experimental group scored better than the control group in diagnosis of gynecologic malignant tumors,case study analysis and total score of the theoretical examination(P<0.05),and the participants in the experimental group were more satisfied than the control group in teaching evaluation in terms of teaching recognition,improvement of gynecologic tumor diagnosis,self-learning ability,follow-up habit and clinical thinking(P<0.05).Conclusion The closed-loop teaching mode of CBL+PBL combined with MDT oriented to“pathology”is more conducive to improving residents'gynecologic tumor ultrasound diagnostic ability and cultivating their clinical thinking and self-learning ability,which is worthy of application and promotion.
