孤立性肺结节患者血清M-CSF、β-Catenin表达水平的相关性及影像学表现 被引量:1

摘要 目的探讨巨噬细胞集落刺激因子(M-CSF)和β-连环素(β-Catenin)在孤立性肺结节(solitary pulmonary nodule,SPN)患者血清中的表达关系及影像学表现特征及其临床意义。方法选取SPN患者5334例,其中实性结节患者2798例、磨玻璃结节患者1787例、混合性结节患者749例作为研究对象,对患者血清M-CSF、β-Catenin表达关系进行分析,并对其与CT影像学表现、SPN癌变风险预测效应进行研究。统计各组复诊影像学表现与癌变发生率并进行比较。结果磨玻璃结节、实性结节、混合性结节患者间血清M-CSF、β-Catenin表达水平比较,差异有统计学意义(P<0.05);混合性结节患者首诊、复诊时血清M-CSF、β-Catenin水平均显著高于其他肺结节组(P<0.05);而实性结节患者复诊与首诊时比较,差异无统计学意义(P>0.05)。经Pearson相关性分析,肺结节患者血清M-CSF与β-Catenin表达水平相互之间表现为显著正相关(P<0.01)。依据影像报告测量基线和CT扫描之间的结节影像学表现,混合性结节与其他肺结节组比较,差异有统计学意义(P<0.01)。各组复诊结果:混合性结节与其他组比较,经病理确诊肺癌比例显著高于其他组,影像学特征无改变比例低于其他组,采用手术治疗的比例显著高于其他组,差异有统计学意义(P<0.01)。结论血清M-CSF、β-Catenin的表达与CT影像学表现,作为SPN肿瘤癌变风险预测的辅助工具,具有重要的临床意义。 Objective To investigate the correlations between the expression of macrophage colony stimulating factor(M-CSF)andβ-Catenin in the serum of patients with solitary pulmonary nodules(SPN),and as well as their imaging features and their clinical significance.Methods A total of 5334 patients with SPN were selected,including 2798 patients with solid nodules,1787 patients with ground glass nodules,and 749 patients with mixed nodules.The correlation between the expression of M-CSF andβ-Catenin was analyzed,and its predictive effect on the risk of SPN carcinogenesis in CT imaging findings was studied.The imaging findings and the incidence of cancerous changes in each group were analyzed and compared.Results The expression levels of serum M-CSF andβ-Catenin in patients with ground glass nodules,solid nodules and mixed nodules were significantly different(P<0.05).There was no significant difference in serum M-CSF orβ-Catenin in patients with solid nodules between initial diagnosis and follow-up examination(P>0.05).Serum M-CSF andβ-Catenin factors in mixed nodule patients were significantly higher than those in other pulmonary nodule groups(P<0.05).The Pearson correlation analysis showed that the expression of serum M-CSF andβ-Catenin in patients with pulmonary nodules was significantly positively correlated(P<0.01).The imaging performance of the nodules between the baseline and CT scan was measured according to the imaging report,there was significant difference in the mixed nodules(P<0.01).There were significant higher cancerous progression and surgery incidences in the mixed nodule group than those in the other 2 groups(P<0.001).Conclusion The expression of serum M-CSF andβ-Catenin,and CT imaging findings have important clinical significance as an auxiliary tool for predicting the risk of canceration of SPN.
