3~10岁龋齿患儿治疗后再次发病影响因素分析及简易评分工具对再次发病预测价值

Analysis of Influencing Factors of Recurrence in 3-10-year-old Children with Dental Caries after Treatment and Predictive Value of Simple Scoring Tool for Recurrence

目的分析3~10岁龋齿患儿治疗后再次发病影响因素及简易评分工具对再次发病预测价值。方法回顾性分析2020年4月-2021年4月收治的3~10岁龋齿患儿200例的临床资料,采用多因素Logistic回归分析对3~10岁龋齿患儿治疗后再次发病影响因素进行分析,绘制受试者工作特征(ROC)曲线对简易评分工具预测3~10岁龋齿患儿治疗后再次发病的价值进行分析。结果多因素Logistic回归分析结果显示,刷牙频率、使用含氟牙膏、变形链球菌情况、主要照顾者口腔知识是否合格和口腔检查频率是3~10岁龋齿患儿治疗后再次发病的影响因素(P<0.01)。将多因素Logistic回归分析结果转化成简易评分工具,总分0~2分提示3~10岁龋齿患儿治疗后再次发病风险低,3或4分提示3~10岁龋齿患儿治疗后再次发病风险高。ROC曲线分析结果显示,简易评分工具预测3~10岁龋齿患儿治疗后再次发病的曲线下面积为0.931(95%CI:0.876,0.967),敏感度和特异度分别为85.71%和89.80%。结论3~10岁龋齿患儿治疗后再次发病影响因素涉及刷牙频率、使用含氟牙膏、变形链球菌情况、主要照顾者口腔知识是否合格和口腔检查频率,据此构建的简易评分工具对3~10岁龋齿患儿治疗后再次发病有可靠预测价值。

Objective To analyze the influencing factors of recurrence of children with dental caries aged 3-10 years after treatment and the predictive value of simple scoring tools for recurrence.Methods The clinical data of 200 children with dental caries aged 3-10 years treated in our hospital from April 2020 to April 2021 were analyzed retrospectively.The influencing factors of recurrence of children with dental caries aged 3-10 years after treatment were analyzed by multivariate Logistic regression analysis.The receiver operating characteristic(ROC)curve of the subjects was drawn to analyze the predictive value of simple scoring tools for recurrence of children with dental caries aged 3-10 years after treatment.Results Multivariate Logistic regression analysis showed that the frequency of tooth brushing,the use of fluoride toothpaste,the situation of Streptococcus mutans,sufficiency of the main caregivers'oral knowledge and the frequency of oral examination were the influencing factors for the recurrence of children with dental caries aged 3-10 years after treatment(P<0.01).The results of multivariate Logistic regression analysis were converted into a simple scoring tool.The total score of 0-2 points indicated that children with dental caries aged 3-10 years had a low risk of recurrence after treatment,and 3-4 points indicated that the risk of recurrence was high in those aged 3-10 years.The ROC curve analysis results showed that the area under the ROC curve of the simple scoring tool in predicting the recurrence of children aged 3-10 years after treatment was 0.931(95%CI:0.876,0.967),and the sensitivity and specificity were 85.71%and 89.80%,respectively.Conclusion The influencing factors for recurrence of children aged 3-10 years with dental caries after treatment involve the frequency of brushing teeth,the use of fluoride toothpaste,the situation of Streptococcus mutans,sufficiency of the oral knowledge of the main caregivers,and the frequency of oral examination.The simple scoring tool constructed on this basis has a reliable predictive value for the recurrence of children aged 3-10 years with dental caries after treatment.
