山东省某地区无偿献血者丙氨酸氨基转移酶筛查不合格情况分析

Analysis on Disqualification of Alanine Aminotransferase of Unpaid Blood Donors Upon Screening in a Certain Area of Shandong Province

目的:分析山东省某地区无偿献血者丙氨酸氨基转移酶(ALT)不合格相关因素,以降低ALT不合格血液报废率。方法:选取2022年5月山东省血液中心街头采血班组的ALT快检下不合格的301例献血者为研究对象,分析这301例献血者的血液标本。结果:301份ALT不合格献血标本中,男258例,女43例;年龄18~24岁46例,25~44岁242例,45~55岁13例。体重≥70 kg 218例,体重<70 kg 83例。因饮食(饮酒、食用高脂肪食品)淘汰105例,占比34.8%;因肥胖(脂肪肝)淘汰93例,占比30.8%;因睡眠不够、熬夜淘汰82例,占比27.2%;药物因素淘汰12例,占比3.9%;其他因素(设备等)淘汰9例,占比2.9%。ALT单项不合格286例,占比95.02%;存在合并梅毒抗体、2型肝炎表面抗原(HBs Ag)、丙型病毒抗体(抗HCV)、抗HIV多项不合格者。结论:本地区ALT不合格的主要为高脂肪饮食、脂肪肝、熬夜的献血者,以男性为主,大部分为ALT单项不合格,也有少数ALT不合格者合并4项传染性指标。

Objective:To analyze the factors related to the disqualification of alanine aminotransferase(ALT) in unpaid blood donors in a certain area of Shandong Province,so as to reduce the rejection rate of ALT disqualified blood.Methods:A total of 301blood donors who failed to pass the ALT fast test by the street blood collection team of Blood Center of Shandong Province in May2022 were selected as the study subjects,and the blood samples of these 301 blood donors were analyzed.Results:Of the 301blood donation samples with disqualified ALT,there were 258 male subjects and 43 female subjects;46 subjects aged 18-24years,242 subjects aged 25-44 years,and 13 subjects aged 45-55 years.218 subjects had a body weight of≥70 kg,and 83subjects had a body weight of <70 kg.105 subjects were eliminated due to diet(alcohol drinking,intake of high-fat food),accounting for 34.8%;93 subjects were eliminated due to obesity(fatty liver),accounting for 30.8%;82 subjects were eliminated due to insufficient sleep and staying up,accounting for 27.2%;12 subjects were eliminated due to drug factors,accounting for3.9%;9 subjects were eliminated due to other factors(equipment and others),accounting for 2.9%.286 subjects were disqualified in single ALT item,accounting for 95.02%;some patients were disqualified due to multiple items among syphilis antibody,Hepatitis B surface antigen(HBsAg),Hepatitis C virus antibodies(anti-HCV),HIV antibodies(anti-HIV) tests.Conclusion:The disqualified ALT patients in this area are mainly blood donors with high-fat diet,fatty liver and staying up;they are mainly male,most of them are disqualified in single ALT item,and a few of them are disqualified in the combination of 4 infectious disease indicators.
