米非司酮联合肌瘤切除术对子宫肌瘤患者的疗效及细胞因子和性激素水平的影响

Effect of Mifepristone Combined with Myomectomy on the Level of Cytokines and Sex Hormones in Patients with Uterine Fibroids

目的分析米非司酮加肌瘤切除术用于子宫肌瘤患者的疗效和对其性激素、细胞因子的影响。方法选取2020年6月至2022年6月郑州市第三人民医院收治的82例子宫肌瘤患者,经随机数字表法将其中41例归纳到对照组(仅予以肌瘤切除术),余下41例归纳到观察组(在前组基础上加以米非司酮)。比较两组在不同治疗方案下的疗效,治疗前后肿瘤坏死因子-α(TNF-α)及白细胞介素-2(IL-2)等细胞因子水平,治疗前后雌二醇(E_(2))、孕酮(P)、促卵泡激素(FSH)及黄体生成素(LH)水平,不良反应出现情况。结果观察组治疗有效率(97.56%)高于对照组(80.49%),差异有统计学意义(P<0.05)。治疗前两组IL-2、TNF-α水平比较,差异无统计学意义(P>0.05),治疗后两组IL-2水平较治疗前升高,差异有统计学意义(P<0.05),TNF-α水平较治疗前降低,差异有统计学意义(P<0.05),且观察组IL-2水平高于对照组,差异有统计学意义(P<0.05),TNF-α水平低于对照组,差异有统计学意义(P<0.05)。治疗前两组E_(2)、P、FSH、LH水平比较,差异无统计学意义(P>0.05),治疗后两组E_(2)、P、FSH、LH水平较前降低,差异有统计学意义(P<0.05),且观察组E_(2)、P、FSH、LH水平低于对照组,差异有统计学意义(P<0.05)。两组不良反应出现情况比较,差异无统计学意义(P>0.05)。结论米非司酮加肌瘤切除术用于子宫肌瘤患者疗效理想,能改善其细胞因子及性激素水平,安全性较高,值得采用。

Objective To analyze the efficacy of mifepristone plus myomectomy in patients with uterine fibroids and its effects on sex hormones and cytokines.Methods A total of 82 patients with uterine fibroids admitted to the Third People’s Hospital of Zhengzhou from June 2020 to June 2022 were selected.By the random number table method,41 cases were assigned to the control group(myomectomy only),and the remaining 41 cases were assigned to the observation group(mifepristone was added to the former group).The efficacy of the two groups under different treatment regimens,the levels of tumor necrosis factor-α(TNF-α),interleukin-2(IL-2)and other cytokines,the levels of estradiol(E_(2)),progesterone(P),follicle-stimulating hormone(FSH)and luteinizing hormone(LH)before and after treatment,and the occurrence of adverse reactions were compared between the two groups.Results The effective rate of the observation group(97.56%)was higher than that of the control group(80.49%),the difference was statistically significant(P<0.05).There was no significant difference in IL-2 and TNF-αlevels between the two groups before treatment(P>0.05);the IL-2 level between the two groups after treatment was higher than that before treatment,the TNF-αlevel was lower than that before treatment,and the difference was statistically significant(P<0.05).The level of TNF-αwas lower than that of the control group(P<0.05),the IL-2 level in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no statistical significance in the levels of E_(2),P,FSH,and LH between the two groups before treatment(P>0.05),but the levels of E_(2),P,FSH,and LH in the two groups after treatment were lower than before,the difference was statistically significant(P<0.05),and the levels of E_(2),P,FSH,and LH in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).There was no statistical significance in the occurrence of adverse reactions between the two groups(P>0.05).Conclusion Mifepristone combined with myomectomy is an ideal therapeutic method for patients with uterine fibroids,which can improve the levels of cytokines and sex hormones.
