小儿肺炎急性期血清IL-6和IL-33及嗜酸细胞趋化因子水平与病情严重程度的相关性研究 被引量:1

摘要 目的探讨小儿肺炎急性期血清白介素(IL)-6和白介素(IL)-33及嗜酸细胞趋化因子(Eotaxin)水平与病情严重程度的相关性。方法选取2021年3月至2022年3月在河南省南阳市中心医院收治的62例急性期肺炎患儿作为研究组,研究组患儿根据其病情严重程度分为轻度组(38例)和重度组(24例),并选取同期缓解期患儿60例作为对照组,比较各组间血清IL-6、IL-33、Eotaxin水平、肺功能指标[用力肺活量(FVC)、最大深呼吸后第1 s呼除气量容积(FEV1)和最大呼气峰流速值(PEF)]水平差异,分析血清IL-6、IL-33、Eotaxin水平对患儿病情程度的评估价值。结果研究组IL-6、IL-33和Eotaxin水平均高于对照组,差异有统计学意义(P<0.05);研究组肺功能指标FVC、FEV1和PEF值均低于对照组,差异有统计学意义(P<0.05)。轻症组IL-6、IL-33和Eotaxin水平均低于重症组,差异有统计学意义(P<0.05);轻症组肺功能指标FVC、FEV1和PEF值均高于重症组患儿的,差异有统计学意义(P<0.05)。IL-6、IL-33、Eotaxin水平变化与小儿急性期肺炎病情程度均呈正相关,差异有统计学意义(r=0.397,0.675,0.317,P<0.05);与肺功能指标FVC、FEV1和PEF值均呈负相关,差异有统计学意义(IL-6:r=-0.456,-0.520,-0.325;IL-33:r=-0.561,-0.511,-0.444;Eotaxin:r=-0.300,-0.385,-0.522,P<0.05)。结论IL-6、IL-33和Eotaxin可能参与着小儿肺炎的发生发展,在急性期呈高水平状态,与肺功能及病情严重程度密切相关,可作为评估病情程度的指标。 Objective To investigate the correlation between serum levels of interleukin(IL-6)and interleukin(IL-33)and eosinophilic chemokines(Eotaxin)and the severity of pneumonia in children at the acute stage.Methods A total of 62 children with acute-stage pneumonia admitted to Nanyang Central Hospital of Henan Province from March 2021 to March 2022 were selected as the study group.According to the severity of their disease,the children in the study group were divided into a mild group(38 cases)and a severe group(24 cases),and 60 children in the remission period were selected as the control group.Serum levels of IL-6,IL-33,Eotaxin,pulmonary function indexes[forced vital capacity(FVC),forced expiratory volume in the first second(FEV1)and maximum peak expiratory fiow rate(PEF)]were compared among all groups.The value of serum levels of IL-6,IL-33,and Eotaxin in evaluating the severity of the disease was analyzed.Results The levels of IL-6,IL-33,and Eotaxin in the study group were higher than those in the control group,the difference was statistically significant(P<0.05).The values of lung function indexes FVC,FEV1,and PEF in the research group were lower than those in the control group,and the difference was statistically significant(P<0.05).The levels of IL-6,IL-33,and Eotaxin in the mild group were lower than those in the severe group,with statistical significance(P<0.05).The values of FVC,FEV1,and PEF in the mild group were higher than those in the severe group,and the difference was statistically significant(P<0.05).The levels of IL-6,IL-33,and Eotaxin were positively correlated with the severity of acute pneumonia in children(r=0.397,0.675,0.317,P<0.05).It was negatively correlated with the values of FVC,FEV1,and PEF,and the difference was statistically significant(IL-6:r=-0.456,-0.520,-0.325;IL-33:r=-0.561,-0.511,-0.444;Eotaxin:r=-0.300,-0.385,-0.522,P<0.05).Conclusion IL-6,IL-33,and Eotaxin may be involved in the occurrence and development of pediatric pneumonia,showing high levels in the acute stage,closely related to lung function and severity of the disease,and can be used as an indicator to evaluate the severity of the disease.
