尿沉渣显微镜法、尿干化学分析仪与全自动尿沉渣分析仪单独及联合进行尿液有形成分评价的性能比较 被引量:4

摘要 目的探究尿沉渣显微镜法、尿干化学分析仪与全自动尿沉渣分析仪单独及联合进行尿液有形成分评价的性能。方法选取2020年1月至2021年12月在新安县中医院行尿液检测的360例患者为研究对象,共收集空腹晨尿样本360份,均采用沉渣显微镜法、URIT-560优利特全自动干化学尿液分析仪及URIT-1500优利特全自动尿沉渣分析仪进行检测,以显微镜检测为“金标准”,比较各检测方式与联合检测方式对尿液有形成分红细胞、白细胞、透明管型检测结果,Kappa一致性分析各检测方式及联合检测评价尿液有形成分的灵敏度、特异度、准确率。结果尿沉渣显微镜法检测结果显示,红细胞(RBC)阳性99例,阴性261例;白细胞(WBC)阳性109例,阴性251例;透明管型阳性103例,阴性257例。尿干化学分析仪检测与全自动尿沉渣分析仪检测结果比较,差异无统计学意义(P>0.05)。三种方式联合检测Kappa一致性分析显示RBC、WBC及透明管型的灵敏度分别为97.98%、99.08%、98.05%,Kappa值分别为0.979、0.993、0.979,三种方式联合检测RBC、WBC及透明管型灵敏度与Kappa值高于尿干化学分析仪检测、全自动尿沉渣分析仪单独检测结果,差异有统计学意义(P<0.05)。结论尿沉渣显微镜法、尿干化学分析仪与全自动尿沉渣分析仪均可良好分析尿液标本有形成分,三种方式联合检测可一定程度提高尿液标本中有形成分红细胞、白细胞检测灵敏度。 Objective To explore the evaluation efficiency of urine sediment microscopy,urine dry chemical analyzer,full-automatic urine sediment analyzer and combined detection for visible urine components.Methods A total of 360 patients undergoing urine examinations in Xin’an County Hospital of Traditional Chinese Medicine between January 2020 and December 2021 were enrolled as the research objects.A total of 360 samples of fasting morning urine were collected.All underwent examinations by sediment microscopy,URIT-560 Uritest full-automatic urine dry chemical analyzer and URIT-1500 Uritest fullautomatic urine sediment analyzer.Taking microscopy as the golden standard,examination results of red blood cells,white blood cells and hyaline casts by different methods were compared.The sensitivity,specificity and accuracy of different methods for evaluating visible urine components were analyzed by Kappa consistency test.Results The results of urine sediment microscopy showed that there were 99 cases with positive RBC,261 cases with negative RBC,109 cases with positive WBC,251 cases with negative WBC,103 cases with positive hyaline casts cast and 257 cases with negative hyaline casts.There was no significant difference in examination results between urine dry chemical analyzer and full-automatic urine sediment analyzer(P>0.05).The sensitivity and Kappa values of combined detection for detecting RBC,WBC and hyaline casts were(97.98%,99.08%,98.05%)and(0.979,0.993,0.979),respectively.The sensitivity and Kappa value of combined detection were higher than those of urine dry chemical analyzer and full-automatic urine sediment analyzer,with significant differences(P<0.05).Conclusion Urine sediment microscopy,urine dry chemical analyzer and full-automatic urine sediment analyzer can all well analyze visible urine components.The combined detection of the three methods can improve detection sensitivity of RBC and WBC to a certain extent.
