水动力清创系统在治疗中重度腋臭中的临床应用

Clinical application of hydrodynamic debridement system in the treatment of moderate to severe bromhidrosis

摘要 目的 评估水动力清创系统在中重度腋臭根治手术中的临床效果和安全性。方法 自2017年1月至2021年12月,空军军医大学唐都医院整形烧伤科收治84例中重度腋臭患者,分别采用小切口负压抽吸联合皮片修剪手术方法(对照组,40例)和小切口负压抽吸联合水动力清创系统手术方法(水刀组,44例)治疗。比较两组手术时长、治愈率、术后并发症发生率(包括局部血肿、感染、皮片坏死、瘢痕、伤口延迟愈合)。结果 水刀组和对照组平均手术时长分别为(52.30±3.16)min和(62.63±5.03)min,差异具有统计学意义(P<0.05);治愈率分别为97.9%和85.0%(P<0.05);两组并发症发生率差异无统计学意义(P>0.05)。结论 采用小切口负压抽吸联合水动力清创系统治疗中重度腋臭,可显著缩短手术时长,提高治愈率,并发症发生率较低,值得临床推广应用。 Objective To evaluate the clinical effectiveness and safety of the hydrodynamic debridement system in radical surgery for moderate to severe bromhidrosis. Methods From January 2017 to December 2021, 84 patients with moderate to severe bromhidrosis were operated by mini-incision negative pressure aspiration combined with hydrodynamic debridement system(waterjet group, 44 cases) and mini-incision negative pressure aspiration combined with skin slice trimming(control group, 40 cases). The total operative time, cure rate and postoperative complication rate(including local haematoma, infection, skin necrosis, scar growth and delayed wound healing) were also compared in terms of the two surgical methods. Results The mean operative time in the waterjet group was(52.30±3.16) min, which was significantly shorter than that in the control group(62.63±5.03) min. The postoperative efficiency rates were 97.9% and 85.0%, respectively, which were statistically different(P<0.05). The difference in the incidence of postoperative complications was not statistically significant(P>0.05). Conclusion The use of mini-incision negative pressure suction combined with hydrodynamic debridement system for the treatment of moderate to severe bromhidrosis has the advantage of significantly reducing the length of the procedure and reducing the recurrence rate after surgery.
