摘要
目的通过西吡氯铵治疗慢性咽炎患者的临床症状分析和对治疗前后咽部黏膜菌群检测的研究,了解西吡氯铵治疗慢性咽炎的功效。方法收集2021年1月~2022年9月期间在我院耳鼻咽喉科门诊就诊的慢性咽炎患者98例,治疗组使用西吡氯铵联合蓝芩口服液治疗,对照组单独使用蓝芩口服液,两组治疗前后通过视觉量表评价咽部主观症状,同时对治疗组进行咽部菌群采样、培养及鉴定。结果治疗组与对照组的总体有效率分别为89.4%和70.8%,治疗组明显高于对照组(P=0.02),治疗组治疗后咽部异物感、咽痒咳嗽、咽干燥感和咽痛感均较治疗前有明显缓解(P<0.01),同时治疗组患者的咽部异物感(P<0.01)、咽痒咳嗽(P<0.01)、咽干燥感(P<0.01)改善明显好于对照组。治疗组47名患者治疗前共检出100例病原菌,检出率前三位的分别是α-溶血性链球菌、肺炎链球菌和流感嗜血杆菌;治疗后共检出64例病原菌,治疗前后比较发现:肺炎克雷伯杆菌、肺炎链球菌、卡他莫拉菌、金黄色葡萄球菌和流感嗜血杆菌的检出率明显下降,奈瑟菌、颗粒链球菌和粪球菌的检出率变化不大。结论西吡氯铵治疗慢性咽炎有效,可引起部分致病菌的检出率下降。
Objective To understand the efficacy of cetylpyridinium chloride(CPC)in the treatment of chronic pharyngitis by analyzing the clinical symptoms of patients with chronic pharyngitis and studying the detection of pharyngeal mucosal flora before and after treatment.Methods A total of 98 patients with chronic pharyngitis treated in the outpatient department of Otorhinolaryngology of our hospital from January 2021 to September 2022 were collected.The treatment group was treated with cetylpyridinium chloride combined with Lanqin oral liquid,and the control group was treated with Lanqin oral liquid alone.The subjective symptoms of the pharynx were evaluated by visual scale before and after treatment,and the pharyngeal flora was sampled,cultured,and identified in the treatment group.Results The overall effective rates of the treatment group and the control group were 89.4%and 70.8%respectively.The treatment group was significantly higher than that of the control group(P=0.02).The pharyngeal foreign body sensation,pharyngeal itching cough,pharyngeal dryness,and pharyngeal pain in the treatment group were significantly relieved after treatment(P<0.01).The pharyngeal foreign body sensation(P<0.01),pharyngeal itching cough(P<0.01),and the improvement of pharyngeal dryness(P<0.01)were significantly better than that in the control group.A total of 100 pathogens were detected in 47 patients in the treatment group before treatment,and the top three detection rates wereα-Streptococcus haemolyticus,Streptococcus pneumoniae,and Haemophilus influenzae.A total of 64 cases of pathogens were detected after treatment.The comparison before and after treatment showed that the detection rates of Klebsiella pneumoniae,Streptococcus pneumoniae,catamora,Staphylococcus aureus and Haemophilus influenzae decreased significantly,while the detection rates of Neisseria,Streptococcus granulosus and fecococcus changed little.Conclusion Cetylpyridinium chloride is effective in the treatment of chronic pharyngitis,which can reduce the detection rate of some pathogenic bacteria.
