两种血管吻合方式在游离皮瓣修复颌面部组织缺损中的对比分析

摘要 目的观察比较微血管吻合器吻合游离皮瓣静脉与手工吻合游离皮瓣静脉之间的差异,分析临床效果。方法选取2019年5月~2021年4月在安徽医科大学第一附属医院与安徽医科大学第二附属医院完成的30例口腔颌面部肿瘤术后缺损行游离皮瓣修复患者(其中一附院15例,二附院15例),动脉采用手工吻合,一附院静脉吻合采用手工吻合,二附院采用吻合器吻合。结果全部皮瓣均成活,微血管吻合器吻合静脉平均用时6分半钟,手工缝合静脉平均用时16分钟,无血管危象发生。结论微血管吻合器可明显缩短吻合时间,减少血管痉挛的发生率,吻合方法相对简单,术后血管通畅度较手工组有所提高。 Objective To observe and compare the difference between free flap vein anastomosis with microvascular coupler and manual anastomosis,and to analyze the clinical effect.Methods 30 patients with oral and maxillofacial tumor were chosen for repair of the defects via free flap(15 cases in the First Hospital,15 cases in the Second Hospital)from May 2019 to May 2021,completed in the First and Second Hospital of Anhui Medical University.Manual method was used in arterial anastomosis.The vein was anastomosed with two methods,one by manual anastomosis in the First Hospital,another by microvascular coupler combined in the Second Hospital.Results All the flaps survived,the average time of vein anastomosis by microvascular coupler was six and a half minutes,and the average time of vein anastomosis by hand was 16 minutes.It was not observed about vascular crisis.Conclusion The microvascular coupler can significantly shorten the operating time and reduce the incidence of vasospasm.Application of microvascular coupler is relatively simple,and the reflux of the anastomosed vein by microvascular coupler went more smoothly than the manual group.
