针刺对穴阳陵泉-太冲治疗急性期肩关节周围炎临床疗效观察

Clinical observation on the treatment of acute periarthritis of shoulder by needling Yanglingquan and Taichong acupoints

目的:评价针刺健侧对穴阳陵泉-太冲配合患侧局部运动治疗急性期肩周炎的临床疗效。方法:将60例入组患者随机分为治疗组(对穴组)和对照组,每组30例。治疗组采用针刺健侧对穴阳陵泉-太冲配合患侧局部运动,对照组采用针刺患侧阿是穴、手三里、肩髎穴、肩髃穴、肩贞穴、外关穴治疗。每天1次,特定电磁波谱(TDP)照射30 min,治疗7 d为1个疗程,共治疗1个疗程。对比两组患者治疗前后活动疼痛程度(VAS评分)和肩关节活动度(Melle评分)并评定两组综合疗效。结果:与本组治疗前相比,两组患者治疗后VAS评分、Melle评分均明显降低(P<0.05),差异有统计学意义;且治疗组VAS评分、Melle评分较对照组改善明显(P<0.05),差异有统计学意义;治疗1个疗程后,治疗组疗效明显优于对照组(P<0.05)。结论:针刺健侧对穴阳陵泉-太冲配合患侧局部运动治疗急性期肩周炎的疗效优于普通针刺。

Objective:To evaluate the clinical effect of Yanglingquan-Taichong acupuncture on the unaffected side combined with local exercise on the affected side in the treatment of acute periarthritis of shoulder.Methods:60 enrolled patients were randomly divided into treatment group(acupuncture group)and control group,with 30 cases in each group.The treatment group was treated with acupuncture at the Yanglingquan and Taichong acupoint on the unaffected side combined with local exercise of the affected side,while the control group was treated with acupuncture at Ashi acupoint,Shousanli acupoint,Jianliao acupoint,Jianyu acupoint,Jianzhen acupoint and Waiguan acupoint.7 d for a course of treatment,a total of 1 course of treatment,1 time per day,TDP irradiation for 30 min.The degree of activity pain(VAS score)and range of shoulder motion(Melle score)before and after treatment were compared between the two groups,and the comprehensive efficacy of the two groups was evaluated.Results:Compared with the pre-treatment period,VAS scores and Melle scores were significantly lower in both groups after treatment,and the difference was statistically significant(P<0.05);VAS scores and Melle scores in the treatment group improved significantly compared to the control group with statistically significant differences(P<0.05);after 1 course of treatment,the efficacy of the treatment group was significantly better than that of the control group(P <0.05).Conclusion:The curative effect of acupuncture at Yanglingquan and Taichong on the unaffected side combined with local movement of the affected side in the treatment of acute periarthritis of shoulder is better than that of ordinary acupuncture treatment.
