基于中医整体观的护理模式对肺癌患者化疗后癌因性疲乏及生存质量的影响

Effects of nursing model based on holistic view of traditional Chinese medicine on cancer~related fatigue and quality of life in patients with lung cancer after chemotherapy

目的探讨基于中医整体观的护理模式对肺癌患者化疗后癌因性疲乏及生存质量的影响。方法选取2020年7月~2022年6月于本院进行化疗的肺癌患者134例,按照随机数字表法分为对照组与观察组,每组67例。对照组采取常规护理模式,观察组采取基于中医整体观的护理模式,所有患者均连续干预2周。比较干预前后2组中医证候积分(恶心呕吐、神疲纳差、夜寐不安)、癌因性疲乏[Piper疲乏修订量表(PFS-R)]、睡眠质量[匹兹堡睡眠质量指数(PSQI)]、生活质量[肺癌患者生存质量量表(QLQ-LC43)]差异。结果干预后两组中医证候积分、PFS-R、PSQI评分较干预前均降低,且观察组低于同期对照组,差异具有统计学意义(P均<0.05);干预后2组QLQ-LC43较干预前均上升,且观察组高于同期对照组,差异具有统计学意义(P均<0.05)。结论基于中医整体观的护理模式能有效改善肺癌患者化疗后症状,减轻癌因性疲乏,提高睡眠及生活质量。

Objective To explore the effects of nursing model based on holistic view of traditional Chinese medicine on cancer~related fatigue and quality of life in patients with lung cancer after chemotherapy.Methods 134 patients with lung cancer who received chemotherapy in the hospital from July 2020 to June 2022 were selected and divided into control group and observation group with 67 cases in each group according to the random number table method.The control group adopted conventional nursing model while the observation group was given nursing model based on holistic view of traditional Chinese medicine,and all patients were continuously intervened for 2 weeks.The differences in traditional Chinese medicine syndromes scores(nausea and vomiting,fatigue and poor appetite,restless sleep),cancer-related fatigue[Piper Fatigue Scale~Revised(PFS~R)],sleep quality[Pittsburgh Sleep Quality Index(PSQI)]and quality of life[Quality of Life Questionnaire for Lung Cancer(QLQ~LC43)]were compared between the two groups before and after intervention.Results After intervention,the scores of traditional Chinese medicine syndromes,PFS~R and PSQI in the two groups were decreased compared with those before intervention,and the scores in observation group were lower than those in control group during the same time period(all P<0.05).The score of QLQ~LC43 after intervention was enhanced in both groups compared to before intervention,and the score in observation group was higher than that in control group during the same time point(all P<0.05).Conclusion Nursing model based on holistic view of traditional Chinese medicine can effectively improve the symptoms of patients with lung cancer after chemotherapy,relieve the cancer~related fatigue,and enhance the sleep quality and quality of life.
