摘要
目的探讨三级综合医院临床护理岗位分级管理的方法。方法于2021年6—7月对盐城市第一人民医院护理人员进行调查。岗位管理小组依据本省岗位管理相关文件要求,确立护理岗位劳动强度、技术难度、风险程度等作为评价指标,采用专家评定法确定各指标权重;以信息化为依托,数据来源于医院HIS系统收费项目及医嘱,同时结合问卷调查的方法,依据能够反映出劳动强度、技术难度的项目进行量化值计算,各护理单元劳动强度量化值为;各护理单元技术难度量化值为。采用李克特(Likert)量表形式对89个护理单元工作风险的风险程度进行调查。最后采用加权秩和比的方法建立岗位分级评价模型。结果根据三项指标将临床科室护理岗位分为五档,其中北区放疗三病区WRSR值最低,划分在第一档,而北区ICU一病区WRSR值最高,划分在第五档。结论利用加权秩和比法对护理绩效进行评价,既可以了解护理关键指标的分类排序,还可以掌握多项指标的综合排序及分档结果,便于护理部掌握全院护理工作的动态信息,对于促进护理学科健康发展具有重要意义。
Objective To explore the method of graded management of clinical nursing positions in tertiary general hospitals.Methods From June to July 2021,the nursing staff of the First People's Hospital of Yancheng City were investigated.The post management team established nursing post labor intensity,technical difficulty,and risk degree as evaluation indexes based on the requirements of relevant documents of post management in our province,and used expert evaluation method to determine the weight of each index;based on informationization,data were obtained from hospital HIS system charge items and medical orders.The quantized value of labor intensity of each nursing unit is∑(reflecting the number of labor intensity items×points/number of nurses in the ward;the quantitative value of technical difficulty of each nursing unit is∑ reflecting the number of items with technical difficulty×points)/number of nursing unit bed days.The Likert scale was used to investigate the risk level of work risks in 89 nursing units.Finally,the weighted rank sum ratio method was used to establish the job grading evaluation model.Results Nursing positions in clinical departments were divided into five grades according to three indicators,among which the lowest W■SR value was in the third ward of radiotherapy in the north zone,which was classified in the first grade,while the highest W■SR value was in the first ward of ICU in the north zone,which was classified in the fifth grade.Conclusion The evaluation of nursing performance using the weighted rank sum ratio method can not only understand the categorical ranking of nursing key indicators,but also grasp the comprehensive ranking of multiple indicators and the grading results.It is convenient for the nursing department to grasp the dynamic information of nursing work in the whole hospital,which is of great significance for promoting the healthy development of nursing discipline.
