筋针疗法治疗急性腰扭伤临床研究

Clinical Study on Tendon Acupuncture in the Treatment of Acute Lumbar Sprain

目的:观察筋针疗法治疗急性腰扭伤的临床疗效。方法:将98例急性腰扭伤患者按照随机数字表法分为观察组和对照组,每组各49例。两组均给予常规治疗,对照组在常规治疗的基础上加用针刺疗法,观察组在常规治疗的基础上加用筋针疗法。比较两组患者的临床疗效及治疗前后腰椎功能[日本骨科协会评估治疗评分(Japanese orthopaedic association, JOA)、oswestry功能障碍指数(oswestry disability index, ODI)评分]、视觉模拟评分法(visual analogue scale, VSA)、炎症因子水平[白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]变化情况。结果:对照组有效率为75.51%,观察组有效率为93.88%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后ODI评分低于本组治疗前,JOA评分高于本组治疗前,且治疗后组间比较,差异具有统计学意义(P<0.05)。两组患者治疗后VAS评分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后IL-6、TNF-α水平均低于治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。结论:筋针疗法治疗急性腰扭伤,可缓解患者疼痛程度,改善腰椎功能,降低炎症反应。

Objective: To observe the clinical efficacy of tendon acupuncture on acute lumbar sprain.Methods: A total of 98 patients with acute lumbar sprain were divided into the observation group and the control group according to random number table, with 49 cases in each group.Both groups were first given routine treatment.Then the control group was given acupuncture on the basis of routine treatment, while the observation group was given tendon acupuncture on the basis of routine treatment.The clinical efficacy, the changes of lumbar function [including Japanese orthopaedic association(JOA),Oswestry disability index(ODI) score],visual analogue scale(VSA),inflammatory factor levels [including interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)] of the two groups before and after treatment were compared.Results: The effective rate was 75.51% in the control group and 93.88% in the observation group.The difference between the two groups was statistically significant(P<0.05).After treatment, the ODI score of the two groups was lower than that of the same group before treatment, while the JOA score was higher than that of the same group before treatment, and all the differences between the two groups were statistically significant(P<0.05);The VAS score of the two groups was lower than that of the same group before treatment, and the observation group′s was lower than that of the control group, and the difference was statistically significant(P<0.05);The levels of IL-6 and TNF-α in two groups was lower than those of the same group before treatment, and the above levels of the observation group were lower than those of the control group, and all the difference were statistically significant(P<0.05).Conclusion: Tendon acupuncture in the treatment of acute lumbar sprain can relieve the pain degree of patients, improve lumbar function and reduce inflammatory reaction.
