透压针刀对冻结期肩周炎肩关节活动度及远期疗效

Point-through-point acupotomy on shoulder joint motion during frozen periodontitis of shoulder and its long-term efficacy observation

目的研究透压针刀对冻结期肩周炎肩关节活动度及对远期临床疗效的影响。方法选取湖南省脑科医院60例冻结期肩周炎患者,按照随机数字表法分为两组:透压针刀组(n=30),传统针刀组(n=30)。两组治疗周期均为每周1次,4次为1个疗程。分别在治疗结束后1 d、治疗结束后1个月、治疗结束后3个月,对比两组患者治疗后的临床疗效、VAS评分、肩关节活动度(ROM)、肩关节功能评分(ConstanMurley)。结果经过治疗后,透压针刀组总有效率显著高于传统针刀组(P<0.05);两组患者VAS评分均显著低于治疗前(P<0.05);并且透压针刀组患者VAS评分显著低于传统针刀组(P<0.05)。两组患者后伸、前屈、内收、外展和外旋ROM均显著高于治疗前(P<0.05);并且透压针刀组患者后伸、前屈、内收、外展和外旋ROM均显著高于传统针刀组(P<0.05)。结论采用透压针刀治疗冻结期肩周炎能够显著提高治疗效果,降低疼痛度,改善患者肩关节功能及活动度,提高冻结期肩周炎康复近远期疗效。

Objective To study the effect of point-through-point acupotomy on the range of shoulder joint motion and its long-term clinical effect on frozen periarthritis of shoulder.Methods Sixty patients with frozen periarthritis of shoulder in our hospital were randomly divided into two groups using the random number table method:point-through-point acupotomyacupotomy group(n=30)and traditional acupotomy group(n=30).The treatment cycle for both groups was once a week,4 times as a course of treatment.The clinical efficacy,VAS,range of shoulder joint motion(ROM)and shoulder function(Constan-Murley)of the two groups were evaluated on the first day after the treatment,the first month after the treatment,and the third month after the treatment.Results After the treatment,the total effective rate of the point-through-point acupotomy group was significantly higher than that of the traditional acupotomy group(P<0.05).The VAS scores of the two groups were significantly lower than those before the treatment respectively(P<0.05).The VAS score of the point-through-point acupotomy group was significantly lower than that of the traditional acupotomy group(P<0.05).The ROM of extension,flexion,adduction,abduction,and external rotation in the two groups were significantly higher than those before the treatment respectively(P<0.05).In addition,the extension,flexion,adduction,abduction and external rotation ROM of patients in the point-through-point acupotomy group were significantly higher than those in the traditional acupotomy group(P<0.05).Conclusion The point-through-point acupotomy is effective in treating frozen periarthritis of shoulder by reducing the degree of pain,improving the function and range of motion of shoulder joint,and elevating the short-term and long-term rehabilitation of frozen periarthritis of shoulder.
