五味甘露药浴颗粒联合塞来昔布治疗类风湿关节炎的临床研究

Clinical study on Wuwei Ganlu Medicated Bath Granules combined with celecoxib in treatment of rheumatoid arthritis

目的 探讨五味甘露药浴颗粒联合塞来昔布胶囊治疗类风湿关节炎的临床疗效。方法 选取2020年8月—2022年5月在焦作煤业集团有限责任公司中央医院就诊的200例类风湿关节炎患者,根据随机数字表法将所有患者分为对照组和治疗组,每组100例患者。对照组患者口服塞来昔布胶囊,1粒/次,2次/d。治疗组在对照组治疗的基础上给予五味甘露药浴颗粒治疗,将1袋药品倒入浴盆,水温控制在40℃,将患病部位浸泡于浴盆内,1袋/次,1次/d,每次浸泡20 min。两组患者连续治疗3周。观察两组的临床疗效,比较两组类风湿关节炎病情评价量表(DAS28)评分、视觉模拟评分法(VAS)评分和血清类风湿因子(RF)、C反应蛋白(CRP)、抗环瓜氨酸肽(CCP)抗体水平。结果 治疗后,治疗组患者的总有效率为94.00%,对照组的总有效率为87.00%,组间比较有明显差异(P<0.05)。治疗后,两组的DAS28评分、VAS评分低于治疗前(P<0.05),且治疗组DAS28评分、VAS评分明显低于对照组,差异有统计学意义(P<0.05)。治疗后,两组的血清RF、CRP、抗CCP抗体水平均显著降低(P<0.05),治疗组的血清RF、CRP、抗CCP抗体水平低于对照组,差异有统计学意义(P<0.05)。结论 五味甘露药浴颗粒联合塞来昔布胶囊治疗类风湿关节炎的疗效确切,能控制病情和减轻疼痛程度,降低RF、CRP、抗CCP抗体水平,并且安全性良好。

Objective To investigate the clinical effect of Wuwei Ganlu Medicated Bath Granules combined with Celecoxib Capsules in treatment of rheumatoid arthritis. Methods Patients(200 cases) with rheumatoid arthritis in Central Hospital of Jiaozuo Coal Industry Group Co., Ltd. from August 2020 to May 2022 were divided into control and treatment groups according to random number table method, and each group had 100 cases. Patients in the control group were po administered with Celecoxib Capsules, 1 grain/time,twice daily. Patients in the treatment group were given Wuwei Ganlu Medicated Bath Granules on the basis of the control group, poured a bag of medicine into the bathtub, control the water temperature at 40 ℃, and soaked the diseased part in the bathtub, 1 bag/time, once daily, soaked for 20 min each time. Patients in two groups were treated for 3 weeks. After treatment, the clinical efficacies were evaluated, and DAS28 scores, VAS scores, the levels of RF, CRP, and anti-CCP antibodies in two groups were compared. Results After treatment, the total effective rate of the patients in the treatment group was 94.00%, and the total effective rate of the control group was 87.00%, and there was a significant difference between two groups(P<0.05). After treatment, the DAS28 score and the VAS score of two groups was lower than that before treatment(P<0.05), and the DAS28 score and the VAS score of the treatment group was significantly lower than that of the control group, with a statistically significant difference(P<0.05). After treatment, the serum levels of RF, CRP, and anti CCP antibodies in two groups were significantly decreased(P<0.05). The serum levels of RF,CRP, and anti CCP antibodies in the treatment group were lower than those in the control group, with a statistically significant difference(P<0.05). Conclusion Wuwei Ganlu Medicated Bath Granules combined with Celecoxib Capsules has definite therapeutic effect in treatment of rheumatoid arthritis, which can control the disease, reduce the degree of pain, and reduce the levels of RF, CRP, and antiCCP antibody, with good safety.
