肺保护性通气序贯肺复张对严重胸外伤致急性呼吸窘迫综合征患者呼吸力学及动脉血气的影响

摘要 目的探讨肺保护性通气序贯肺复张治疗对严重胸部外伤致急性呼吸窘迫综合征(ARDS)患者呼吸力学及动脉血气的影响。方法选取2018年8月至2019年8月该院收治的严重胸外伤致ARDS患者118例,随机分为观察组与对照组各59例。对照组予小潮气量机械通气联合最佳呼气末正压通气,观察组在机械通气基础上予肺保护性通气序贯肺复张治疗。观察两组治疗前与治疗12天后呼吸力学与血气指标变化。结果治疗后观察组气道峰压、pH值低于对照组,肺静态顺应性、动脉血氧分压、氧合指数高于对照组,差异均有统计学意义(t=11.61、2.41、6.33、6.05、13.79,P<0.05)。结论肺保护性通气序贯肺复张治疗,能显著改善严重胸部外伤致ARDS患者呼吸力学与血气指标。
