跨理论模型综合干预对甲状腺癌围手术期患者负性情绪及疾病不确定感的影响

Influence of transtheoretical model comprehensive intervention on negative emotion and disease uncertainty of perioperative thyroid cancer patients

目的:探讨跨理论模型综合干预对甲状腺癌围手术期患者负性情绪及疾病不确定感的影响。方法:将2020年4月-2021年4月于某院就诊的甲状腺癌患者100例随机分为两组。对照组50例予以常规干预,观察组50例在对照组的基础上加以跨理论模型综合干预。比较两组患者干预前后负性情绪评分、疾病不确定感、应对方式、生活质量以及干预满意度。结果:观察组患者干预后抑郁(SDS)评分明显低于对照组(t=2.607,P<0.05);观察组患者干预后MUIS量表各维度评分低于对照组(t=3.738,4.950,4.580,5.396;P<0.05);观察组干预后积极应对评分高于对照组,消极应对评分低于对照组(t=5.356,3.370;P<0.05);观察组干预后GQOL-74量表各维度评分明显高于对照组(t=3.037,2.014,4.281,2.577;P<0.05);观察组患者干预总满意度为94.00%明显高于对照组的78.00%(χ^(2)=5.316,P<0.05)。结论:跨理论模型综合干预有利于缓解甲状腺癌围手术期患者的负性情绪,降低疾病不确定感,改善应对方式,提高生活质量和干预满意度。

Objective:To discuss the influence of transtheoretical model comprehensive intervention on negative emotion and disease uncertainty of perioperative thyroid cancer patients.Methods:A total of 100 thyroid cancer patients admitted to our hospital from April 2020 to April 2021 were selected and were divided into observation group and control group with 50 cases in each group according to the random picking method.The control group received routine intervention,while the observation group received transtheoretical model comprehensive intervention on the basis of the treatment of the control group.The negative emotion score,disease uncertainty,coping style,quality of life and intervention satisfaction degree were compared between the two groups before and after intervention.Results:After intervention,the scores of self-rating depression scale(SDS)in the observation group were significantly lower than those in the control group(t=2.607,P<0.05);After intervention,the scores of unpredictability,uncertainty,information lack and complexity of the observation group were significantly lower than those of the control group(t=3.738,4.950,4.580,5.396;P<0.05);After intervention,the positive coping score of the observation group was significantly higher than that of the control group(t=5.356,P<0.05),and the negative coping score was significantly lower than that of the control group(t=3.370,P<0.05);After intervention,the scores of social function,psychological function,physical function and material life in the observation group were significantly higher than those in the control group(t=3.037,2.014,4.281,2.577;P<0.05);And the total intervention satisfaction degree of the observation group(94.00%)was significantly higher than that of the control group(78.00%)(χ^(2)=5.316,P<0.05).Conclusion:Transtheoretical model comprehensive intervention is beneficial to alleviating the negative emotion of perioperative thyroid cancer patients,reduce their disease uncertainty,improve their coping style and improve their quality of life and intervention satisfaction degree.
