摘要
目的 探讨巨噬细胞移动抑制因子(MIF)、伴有kazal域的富含半胱氨酸的逆转诱导蛋白(RECK)及外周血循环肿瘤细胞(CTC)在骨肉瘤中的表达及临床意义。方法 将132例骨肉瘤患者设为观察组,另选取同期的129例软骨瘤患者设为对照组。对比两组患者MIF、RECK、CTC阳性表达情况;对观察组患者随访2年,分析不同预后骨肉瘤患者MIF、RECK、CTC阳性表达情况;采用Cox多元回归分析对骨肉瘤患者预后影响因素进行分析。结果 观察组患者MIF、CTC阳性表达率均明显高于对照组,RECK阳性表达率明显低于对照组,差异均有统计学意义(P﹤0.01)。随访2年,132例骨肉瘤患者中,死亡58例,生存74例。死亡患者MIF、CTC阳性表达率均明显高于生存患者,RECK阳性表达率明显低于生存患者,差异均有统计学意义(P﹤0.01)。发生肺部转移、低中分化、MIF阳性、RECK阴性、CTC阳性均为骨肉瘤患者预后死亡的危险因素(P﹤0.05)。结论 MIF、CTC、RECK可用于骨肉瘤患者早期诊断及预后评价中,能作为评估骨肉瘤患者预后的标志物。
Objective To investigate the expression and clinical significance of macrophage migration inhibity factor(MIF), reversion inducing cysteine rich protein with kazal motifs(RECK), and circulating tumor cell(CTC) in osteosarcoma. Method A total of 132 patients with osteosarcoma were selected as the observation group, and 129 patients with chondroma during the same period were selected as the control group. The positive expressions of MIF, RECK, and CTC were compared between the two groups. The patients in the observation group were followed up for two years, and the positive expressions of MIF, RECK, and CTC in osteosarcoma patients with different prognoses were analyzed. Cox multiple regression analysis was used to evaluate the prognostic influence factors for osteosarcoma patients. Result The positive expression rates of MIF and CTC in the observation group were significantly higher than those in the control group,and the positive expression rate of RECK was significantly lower than that in the control group, and the differences were statistically significant(P<0.01). During the 2-year follow-up, among 132 patients with osteosarcoma, 58 died and 74 survived. The positive expression rates of MIF and CTC in dead patients were significantly higher than those in surviving patients, and the positive expression rate of RECK was significantly lower than that in surviving patients, and the differences were statistically significant(P<0.01). Lung metastasis, low-middle differentiation, MIF positive, RECK negative, and CTC positive were risk factors for prognosis in patients with osteosarcoma(P<0.05). Conclusion MIF, CTC, and RECK could be used for early diagnosis and prognosis evaluation of osteosarcoma patients, and have the potential to be used as markers for evaluating the prognosis of osteosarcoma patients.
