类风湿关节炎患者乐观心理水平影响因素及其与生活质量的相关性分析

Correlation between optimism and quality of life in patients with rheumatoid arthritis

目的:了解类风湿关节炎(RA)患者乐观心理水平和生活质量的现状,分析RA患者乐观心理水平的影响因素及乐观心理水平与生活质量的关系。方法:采用方便抽样法,选取2021年11月—2022年1月山西医科大学第二医院风湿免疫科就诊的220例RA患者为研究对象,采用一般资料调查表、生活定向测验修订版(LOT-R)和简明健康状况调查量表(SF-36)进行调查,采用Pearson相关分析和线性回归分析探讨RA患者乐观心理水平的影响因素及乐观心理水平和生活质量的关系。共发放问卷240份,回收有效问卷220份,有效回收率为91.7%。结果:220例类风湿关节炎患者乐观心理水平得分为(14.55±3.52)分,处于中等偏下水平;RA患者的生活质量各维度得分均低于国内常模;不同疾病活动度、疼痛、社会支持程度的RA患者乐观心理水平得分差异有统计学意义(P<0.05);RA患者乐观心理水平与生活质量中总体健康、活力、社会功能、精神健康4个维度得分呈正相关。结论:RA患者乐观心理水平与生活质量得分较低,医护人员应重视患者的心理状态及生活质量,帮助患者建立科学有效、乐观的促进干预渠道和疾病管理模式,提高患者的乐观心理水平与生活质量。

Objective: To investigate the status of optimism and quality of life in Rheumatoid Arthritis(RA) patients, and to analyze the influencing factors and the relationship between optimism and quality of life in RA patients.Methods: In this study, 220 RA patients admitted to the rheumatology department of the second hospital of shanxi medical university from November 2021 to January 2022 were selected by convenient sampling method.General data questionnaire, revised Version of the Life Orientation Test(LOT-R) and Medical Outcomes Study Short Form 36(SF-36).Pearson correlation analysis and linear regression analysis were used to explore the influencing factors of optimism level and the relationship between optimism level and quality of life in RA patients.A total of 240 questionnaires were sent out, and 220 were effectively received with effective recovery of 91.7%.Results: The positive psychological level score of 220 rheumatoid arthritis patients was(14.55±3.52),which was lower than the middle level.The scores of all dimensions of quality of life in RA patients were lower than the domestic norm.There were statistically significant differences in the scores of optimism level among RA patients with different education, occupation and social support(P<0.05).The level of optimism of RA patients was positively correlated with the scores of overall health, vitality, social function and mental health in quality of life.Conclusion: The scores of optimism and quality of life in RA patients are low, so medical staff should pay attention to the psychological state and quality of life of patients, help patients establish scientific and effective intervention channels for optimism promotion and disease management mode.
