溃疡性结肠炎患者对粪菌移植的认知及接受度研究

Cognition and Acceptance of Fecal Microbiota Transplantation in Patients with Ulcerative Colitis

目的 了解溃疡性结肠炎(UC)患者对粪菌移植(FMT)的认知及接受度。方法 选取2018年6月至2019年6月于新乡医学院第一附属医院就诊的120例UC患者作为研究对象,以问卷的方式进行调查。结果 120例患者中,78例(65.00%)不了解FMT,42例(35.00%)了解FMT。两组患者年龄和受教育程度比较,差异有统计学意义(P<0.05);两组其他资料比较,差异无统计学意义(P>0.05)。logistic分析结果显示初中及以上受教育程度患者更了解FMT。大部分患者主要通过网络途径了解FMT,认为FMT恶心但是有效;患者接受FMT的主要原因是其他治疗方法无法控制症状;粪便标本检测不充分或者处理不规范导致疾病传染是最令患者担忧的问题;患者倾向于选择医生推荐的人作为供体,选择通过肠镜或灌肠接受FMT;如果FMT效果差,仍有难治性UC患者选择再次接受治疗。结论 大多数接受调查的UC患者不了解FMT,初中及以上受教育程度患者更了解FMT,患者对FMT本身的卫生安全以及可能传染疾病存在担忧。

Objective To understand the cognition and acceptance of patients with ulcerative colitis(UC) to fecal microbiota transplantation(FMT).Methods A total of 120 patients with UC who were treated in the First Affiliated Hospital of Xinxiang Medical College from June 2018 to June 2019 were selected as the study subjects and investigated by questionnaire.Results Among the 120 patients, 78 cases(65.0%) did not know FMT, and 42 cases(35.0%) knew FMT. The age and education level between the two groups were different(P<0.05). There was no difference between the two groups in other data(P>0.05). Logistic analysis showed that patients with junior high school education and above were more familiar with FMT. Most patients learned about FMT mainly through the internet, and thought FMT was disgusting but effective. The main reason for patients receiving FMT was that other treatment methods can not control symptoms. Inadequate detection of stool samples or nonstandard treatment leading to disease transmission was the most worrying problem for patients. Patients tended to choose the person recommended by the doctor as the donor and choose to receive FMT through colonoscopy or enema. If the effect of FMT is poor, there were still patients with refractory UC who choose to receive treatment again.Conclusion For the majority of the persons with UC surveyed were unaware of FMT, and the patients with junior high school education or above know FMT better. The patients have concerns about the health and safety of FMT itself and possible infectious diseases.
