摘要
目的 研究早期乳腺癌乳腺X射线摄影中不同类型微小钙化灶与患者临床病理特征的关系。方法 回顾性分析2018年7月至2022年3月于郑州大学附属郑州中心医院接受乳腺X射线摄影检查,且确诊为早期乳腺癌的58例患者的完整临床资料。以乳腺X射线摄影的影像中是否出现微小钙化灶为标准,将样本分为微小钙化灶组(35例)和非微小钙化灶组(23例)。分析微小钙化灶与早期乳腺癌临床病理特征的相关性。结果 两组年龄、部位、乳腺影像报告和数据系统(BI-RADS)分级、TNM分期、肿瘤病理类型、雌激素受体(ER)、孕激素受体(PR)比较,差异无统计学意义(P>0.05),两组人表皮生长因子受体2(HER2)表达比较,差异有统计学意义(P=0.015);不同ER、HER2表达的泥沙状钙化类型比较,差异有统计学意义(P=0.040,P=0.040),不同PR表达的泥沙状钙化类型比较,差异无统计学意义(P>0.05);不同ER、PR及HER2表达的小杆状钙化、小叉状钙化比较,差异无统计学意义(P>0.05)。结论 HER2阳性与微小钙化灶的出现有关;ER阳性和HER2阳性均与泥沙状钙化的出现有关。
Objective To investigate the relationship between different types of microcalcifications in early breast cancer mammography and clinical pathological characteristics of patients.Methods The complete clinical data of 58 patients who were diagnosed with early breast cancer and accepted mammography examination in Zhengzhou Central Hospital Affiliated to Zhengzhou University from July 2018 to March 2022 were retrospectively analyzed. Taking the presence of microcalcifications in mammography as the standard, the patients were divided into microcalcification group(35 cases) and non-microcalcification group(23 cases). The correlation between microcalcifications and clinicopathological features of early breast cancer was analyzed.Results There were no statistical differences in age, location, breast imaging reporting and data system(BI-RADS) grade, TNM stage, tumor pathological type, estrogen receptor(ER), progesterone receptor(PR) between two groups(P>0.05). There was statistical difference in human epidermal growth factor receptor 2(HER2) expression between two groups(P=0.015). There was statistical difference in the types of sediment-like calcification expressed by different ER and HER2(P=0.040, P=0.040), but there was no statistical difference in the types of sediment-like calcification expressed by different PR(P>0.05). There were no statistical differences in small rod-shaped calcification and small fork-shaped calcification of different ER, PR and HER2 expressions(P>0.05).Conclusion The appearance of microcalcifications in mammography are associated with the expression of HER2. The appearance of sediment-like calcifications are associated with expression of ER and HER2.
